Employee was trying to take a belt off motor. He thought the motor was stopping. When the belt popped off, the motor gained momemtum throwing the pry bar into employee's face causing a laceration. He received 20 stitches.

Employee was shoveling under conveyor when a rock popped over the top hitting hom on the head.

EMPLOYEE WAS REPAIRING MIRROR ON EUCLID, SLIPPED AND FELL OFF TRUCK INJURING HIS LEFT WRIST.
