Employee reported that a handlebar from the "Stop Sign" cart fell and hit EE's knee when EE was moving it into position in front of a truck. The handlebar is welded into position so the event, as described, could not be verified.

Recent CT Scan of lungs indicated calcified Plural plaques on lungs. Prior long term employment at the Libby MT Vermiculite mining operation is the likely result of known asbestos exposures at this mine

Employee was acting as a spotter to position a pipe for another employee operating a forklift to lift a section of tubing through the doorway on the 2nd fl, of mill. During the process the pipe contacted the metal stair handrail causing the pipe to swing around and contact the employee who received a small cut from the shield edge of his safety glasses.

Employee was lifting (with help) a battery into rear of pick-up truck. Felt pain in his left shoulder. Reported to supervisor immediately. Final Medical treatment was surgery on 2/5/2010 to repair tendon in shoulder.

Employee was changing worn bits on a cultivator at a reclamation mine project using an air impact wrench to loosen restraining nut. He supported himself with left hand holding on to back part of bit, the flat head bolt had a barb on it, and it spun around causing a small circular cut in palm of hand.

Two millwrights were taking a hose sweep from the warehouse wall rack to the floor, so they could move it with a forklift. As they pulled it off the rack, one end at a time, one employee felt a pain in his back. Appeared to be a back sprain.

Employee was attaching a water hose to a pumping station to fill up the Mine Water truck. Due to a divot in the ground from water leakage, his left foot stepped into the divot, causing his leg to buckle under him. In an effort not injure himself further, he rolled on the ground in the direction of the left leg

FRAGMENT FLEW OFF FROM THE TOOTH SOME DISTANCE FROM THE HAMMER STRIKE EE IN HIS LEFT THIGH.

EE STEPPED ON GRATING AT MONITOR CLAMSHELL AREA. THE GRATING SLIPPED OFF ONE EDGE, CAUSING HIM TO FALL WITH IT INTO THE DITCH. HE FELL ONTO A CORNER OF THE STEEL GRATING, WHICH LACERATED THE B ACK OF HIS LEFT INNER THIGH. HE CLIMBED OUT OF THE DITCH. FIRST AID WAS APPLIED, AND AN AMBULANCE WAS SUMMONED. ER DR.'S CARE-RELEASED 1.5 HOURS LATER.

EMPLOYEE HAD JUST COMPLETED REPAIR WORK ON THE CONCENTRATE BELT FEEDER. AS HE WALKED AWAY FROM UNIT AFTER COMPLETION OF HIS REPAIR, HIS LEFT FOOT FELL THROUGH METAL FLOORING. AS HE EXTENDED HI S HANDS, IT CAME IN CONTACT WITH A PIECE OF METAL PATCHING WITH JAGGED EDGES.

LATER IN THE SHIFT HIS BACK WAS VERY SORE. DR'S ANALYSIS STATED A SLIGHT STRAIN/SPRAIN OF THE LOWER BACK.

HE WAS HOLDING THE UNIT HALFWAY FROM HISBODY. AFTER HE FELT A TWING, HE HELD THE POSITION TILL UNIT WAS SECURED.

EMPLOYEE HAD ENERGIZED THE GATE TO THE ROCK HOPPER TRUCK. AS HE WAS WAITING FOR IT TO CLOSE, HE PICKED UP A WATER HOSE TO WASH DOWN THE AREA. AS HE DID SO HE BACKED UP AND TRIPPED ON A SECONDA RY HOSE IN THE AREA CAUSING HIM TO FALL BACKWARDS QUITE HARD.
