Employee reported that a handlebar from the ""Stop Sign"" cart fell and hit EE's knee when EE was moving it into position in front of a truck. The handlebar is welded into position so the event, as described, could not be verified.

Employee was opening a door, decided not to, two other employees open door causing employee to lose his balance. Employee fell to the floor and hurt his back.

Employee was lifting (with help) a battery into rear of pick-up truck. Felt pain in his left shoulder. Reported to supervisor immediately. Final Medical treatment was surgery on 2/5/2010 to repair tendon in shoulder.

EMPLOYEE WAS REACHING TO MOVE A PIECE OF MEAD BOARD (CARDBOARD) FROM MOVING ROLLERS WHEN HIS RIGHT MIDDLE FINGER GOT CAUGHT BY ROLLER CUTTING THE FINGER REQUIRING 5 STITCHES.

PASSING BY CONVEYOR, NOTICED BAGS WERE BACKED UP AND FALLING OFF CONVEYOR. WALKED AROUND TO TURN OFF SWITCH, STEPPED ON BAG AND TWISTED ANKLE.

EE WAS IN THE PROCESS OF DISMOUNTING THE PC300, WHEN HE PLACED HIS FOOT ON THE GROUND. HE STEPPED ON A LUMP OF KAOLIN, WHEN THE LUMP ROLLED, HE TWISTED HIS KNEE.

EMPLOYEE WAS INSTALLING WATER PUMP ON PL-E TRYING TO LINE UP BOLT HOLES, FELT SOMETHING PULL IN BACK OF NECK. EMPLOYEE TRATED HIMSELF UNTIL 8/27 THEN WE TOOK HIM TO THE DOCTOR.

EMPLOYEE WAS TIGHTENING A DRILL BIT ON DRILL PROD. THE CHUCK KEY SLIPPED CAUSING EMPLOYEE TO HIT KNUCKLE TO HIT THE DRILL BIT AND CUTTING THE KNUCKLE.
