Miner was leaving the office and traveling down a flight of stairs when they realized that they had forgotten something. Turned in the middle of the stairs and lost balance and fell scraping left shin on a boot scrubber mounted on the bottom step.

On 1/2/2025 employee was removing QR caps from a rotary crusher. As EE was removing one of the caps it fell before EE had a good grip smashing EE's right middle finger between the cap and housing. EE received 4 stitches and also a fracture. Employee was on restricted duty for three weeks. Other workers were in the area. Communication and readiness were contributing factors.

Pit Foreman was climbing up on the off-side of a Volvo L-250 Rubber tire loader to tighten a bolt on the tension spring to scaler. While stepping to designated location, EE's right foot slipped and caused EE to fall to the ground where EE landed on their back. EE was transported to the hospital where x-rays revealed EE fractured the L1 vertebrate.

On 5/12/2023 at approximately 4:40pm EE was removing a trough roller from under a crusher plant. While EE was pulling the roller EE mashed right thumb between the roller frame and the frame of the crusher. EE was treated at the ER and received 2 stitches and a fracture to a bone in thumb. EE was put on restricted duty.

EE reported on 11/14/2022 that EE had been injured on 11/10/2022. EE was working setting a chute, when EE shifted EE's weight on EE's legs and heard something pop in EE's right hip. EE was advised to seek medical treatment but declined. On 11/22/2022 EE sought out medical attention. EE has been referred for an MRI, diagnosed suspect labral re-injury.

Miner was working the maintenance crew installing a belt on a conveyor. EE was threading a belt around a tail pulley. While pulling on the belt the miner strained left hand causing swelling and pain.

Miner was assisting in a belt install on a conveyor. The belt was being pulled on with a rope. The miner placed a gloved hand on the head pulley to get a look at the progress. The rope broke under tension and struck the miner in the right hand. Miner sustained a fracture to the thumb of the right hand.

Employee was driving a CAT 775 rigid frame truck from stock pile. While being loaded the truck was hit with the bucket of 988 CAT loader. Employee said they felt something back pop. There was no immediate pain but over a short period of time they felt pain in their back.

Employee was changing the bearing on a shaft using a chisel and hammer to remove race. Hammer glanced off chisel hitting left index finger near the nail. Smashed and cut left index finger.

On 9/19/2019 EE reported at 10:00am they exited a loader put on a harness and went up on to a conveyor to clear off some rocks from a belt and a screen feeder. EE said they tweaked their back. EE called to report incident at 1:45. EE told that they had been having issues with back at home and wasn't sure where the injury happened.

On 5/24/2019 at approximately 3:45pm. EE was power washing rubber tire loader when EE over extended the wand causing it pull back causing the wand to travel across EE's boot cutting boot and causing 2nd degree burn to right foot.

EE was trying to install a CB radio antenna on a pickup truck using a knife to cut zip ties. Knife slipped cutting the left index finger. Knife cut through the finger nicking the middle finger. EE went to emergicare who said the cut was too deep that EE needed to go to the emergency room. Received 4 stitches, antibiotics and tetanus shot.

EE was washing the cab of the 740 truck. While exiting the cab ee reported that ee was using three points of contact and foot slipped out. One foot was up on the second rung and the other on the ground. EE said that ee initially didn't feel any pain, but as the day progressed driving the truck ee back started hurting.

While working on screen conveyor at lime shed ee tried to catch a falling piece of pipe, that had been hit by manlift above ee to keep it from hitting another employee. Pipe slipped through ee hand causing laceration next to thumb.

Plant operator was helping with a screen change. EE was tightening bolts with right hand and had EE's left arm resting on screen. Being that EE had previously dislocated EE's shoulder in April of 2016, it was weak and again slipped out of socket. this is a reoccurring injury that did not result in lost time.

Employee was conducting a maintenance action on the DBT Crusher. Employee was helping with moving cribbing blocks from the tail pulley of feeder chain and was reaching up when the chain settled and shifted. When employee planted foot it slipped approximately 1 ft. Employee reached up and over head causing shoulder to dislocate.

Employee was changing a grease hose when EE slipped and fell, injuring EE's left shoulder. EE was taken to an emergency and the Employee was treated at the hospital where EE was diagnosed with a separated shoulder and treated by mobilization. Employee was released to return to EE's next scheduled shift.

While backing up the truck to dump, employee stated that the truck downshifted, thus hurting employee's back. Employee was diagnosed with a pulled muscle and missed two days of work.

Released back to work with a "sitting only" restriction. Tem 8F. Pit excavator drove fuel truck to machine in pit bottom & park the truck. Exited to chock the rear wheels. EE's right foot slipped on an ice puddle EE caught EE's full weight on EE's left foot. EE experienced pain in EE's ankle. RMH/Sentara Corp health determined EE's injury to be Achilles Tendon Entesopathy.

Miner was attempting to cut the bearing out of a pulley assembly on C12 while plant was down for a complete conveyor overhaul when his right forearm came into contact with the acetylene torch he was utilizing. Miner suffered second degree burn. Foreman administered first aid. Miner was taken to urgent care facility for treatment.

Heavy Equipment Operator was operating an excavator in the sump area of the pit-complained of shortness of breath/minor chest pain. Initial FA/triage was conducted by plant manager. PT was assessed to be having a heart attack, called 911/ EMS. PT was evacuated by EMS to local hospital. PT was not having heart attack, just an asthma attack. PT was released from hospital same day.

SNI was cleaning up after finishing changing the conveyor on our #3 belt. He was carrying a board back to the service truck. As he placed the board onto the back of the truck, the back of his right hand made contact with a belt re-grooving tool that was cooling- leaving a 2 inch laceration.

Said Named Individual (SNI) was preparing to replace the bags in the Sly Baghouse. He had just opened the door to the bag area, leaned down to latch the door when the wind caught the door. The door closed on SNI's left arm and hand. SNI lifted the door and notified foreman of incident. SNI was diagnosed with a contusion to his left forearm.

As Contractor backed his tool truck, he struck the bucket of the Excavator. Injured EE was located next to the bucket and was pinched between the service truck and the 345B bucket. EE was transported by EMS to the Hospital. The injury resulted in a completely serparated Pelvis.

Employee mentions that he was placing processed stone on to pallets and felt a quivering in the center of his lower back before it gave out on 6/12/07. The employee thought he was simply tired and sore. After rest and over a week of time his back was still sore with pain extending down his right leg. Incident reported on 6/21/07 to a Foreman.

Employee was standing outside of his assigned CAT 771D Off - Road Haul Truck and mentioned that a gust of wind blew some fugitive dust around which entered his right eye. He reported the incident to a Foreman on Monday, 6/4/2007. He was treated by a physician and a corneal abrasion of the right eye was diagnosis. Employee was given antibiotics.

EE was reaching from the exit belt side of the rock splitter machine underneath of the veneer gauge to remove a stone and mentions that his right pinkie finger was smashed between a rock being processed by the machine and the veneer gauge. *NOTE* He was wearing his leather gloves at the time of the incident.

EMPLOYEE WAS OPERATING A HAUL TRUCK ON 9/9/05 & A SMALL ROCK FELL FROM THE BED OF HIS TRUCK. HE EXITED HIS TRUCK & ATTEMPTED TO ROLL THE ROCK FROM THE HAUL ROAD WHEN HE EXPERIENCED A STINGING PAIN IN HIS GROIN AREA. HE IMMEDIATELY REPORTED THE INCIDENT TO FOREMAN. OPERATOR WAS TIGHTENING BOLTS WITH ANOTHER EMPLOYEE ON THE CRUSHER WITH A MANUAL TORQUE WRENCH. THE TORQUE WRENCH BROKE AND CAUSED HIM TO STUMBLE BACKWARDS AND TWIST HIS ANKLE, LEFT LEG. HE WORKED THE REMAINDER OF THE SHIFT AND WAS SEEN BY A DOCTOR ON 5/27/04 DUE TO ANKLE PAIN.

TRUCK DRIVER WAS DESCENDING THE FRONT LADDER OF THE TRUCK. HIS FOOT SLIPPED OR HE MIS-JUDGED THE STEP AND FELL ABOUT 4 FEET TO THE GROUND, LANDING ON HIS FEET. WORKED THE REMAINING SHIFT BUT H AD TO BE SEEN AT DOCTOR THE NEXT DAY BECAUSE OF PAIN & STIFFNESS IN THE LEFT KNEE. LOST TIME. *NOTE* THIS OPERATOR WAS FACING THE LADDER PROPERLY WHEN THIS OCCURRED.*

MECHANIC WAS ASSISTING THE BOBCAT OPERATOR IN REMOVING THE BUCKET. AFTER THE PINS WERE RELEASED AND THE OPERATOR STARTED TO BACK AWAY, HE ACCIDENTLY MOVED THE CONTROL THE WRONG WAY CAUSING THE EDGE OF THE BUCKET TO LIFT. THE MECHANIC WASSTILL TOO CLOSE AND THE BUCKET EDGE STRUCK HIS LEFT LEG.

EE WAS CUTTING SECTIONS OF CONVEYOR BELT W/UTILITY KNIFE. HE HAD TO BEND OVER A LOT AND PHYSICALLY HANDLE THE SECTIONS. THE SECTIONS WERE 10' LONG AND 16" WIDE. THE REPETITION OF THIS MAY HAVE CAUSED THE STRAIN, ALTHOUGH EE IS NOT CERTAIN OF IT.
