A PRIMARY CRUSHER OPERATOR WAS FATALLY INJURED WHEN HE WAS STRUCK BY THE BUCKET OF A FRONT-END LOADER. THE VICTIM ENTERED THE CRUSHER FEED HOPPER AND WAS STRUCK BY THE BUCKET WHEN THE LOADER OPERATOR STARTED DUMPING MATERIAL INTO THE HOPPER, NOT KNOWING THE VICTIM WAS IN THE HOPPER.
