The employee was tasked with helping fill water tank. EE was found about 30 minutes later laying face down saying EE was hurt and needed help. At this point it appears that the weight of the trailer shifted off the cribbing under the landing gears and caused the trailer to overturn, either striking against or rolling over the miner.

Employee was setting timbers for the next retreat mining lift inby spad 21417 when the slab cut entry that was previously cut fell to the roof bolts causing draw rock to strike the employee. Employee was coherent and alert while being transported outside. During transportation to medical facility they suffered a cardiac arrest. Incident remains under investigation.

A contractor died after a concrete retaining wall collapsed into the excavated trench below crushing EE.
Nobody saw the accident and we have not been able to interview the EE
Third party delivery driver of raw materials was found deceased trapped between the frame and dump bed of EE's I-Bar owned and operated delivery truck. The driver was located on the West edge of the iron ore stockpile.
During preparation for dismantling of motor and gear box on slide gate. Employee didn't follow given instructions and removed all bolts from gearbox. That cause released of stored energy on slide gate and motor with gear box start spinning. Employee got caught by the equipment and started spinning with it. During spinning their head hit the handrail which caused a fatal injury.
The injured employee was performing maintenance on a rock crusher using a cutting torch when a fire occurred. The injured employee received burns to EE's body and which ultimately proved fatal.
The employee stepped out of manhole and onto the slope and was struck by a slope tractor traveling down the slope.

The employee was pressure washing silo #6 and was found with their head and right arm in the silo with salt buildup on their head and right arm. The employee was pronounced dead at the scene.
A water inundation occurred on the No. 1 Section in the No. 1 Face near spad 1717. Resulted in a Fatal injury. Rescue crews located the fatally injured person on 11/13/2025.
The victim was fatally injured when ejected from the operator's compartment of a diesel scoop after the scoop was struck by a locomotive that had lost control of a trip of supplies along the 41 Butt track haulage.
For unknown reason, haul truck veered over the berm and into quarry lake.

Miner was on first floor of "New Office" working on a ladder changing a light. Miners co-worker went to basement to retrieve another new light. Co-worker heard a thump and a bang on the ceiling. Co-worker found miner sitting slumped on the floor unresponsive. Started CPR, called 911, deployed AED but the miner could never be revived.
IDS driver was instructed by Lynch to dump employee's load under unsafe conditions. The berm that the Lynch operator constructed was inadequate/non-existent due to the pile being dug into by Lynch from below. This caused the earth to fall away under back tires. End dump rolled down embankment flipping and landing on its roof. Employee was pronounced dead at the hospital.
The EE was loading lifters when a piece of rock (approx. 4'x 5' x 2') fell from the ceiling and struck the EE. The EE was not pinned under the rock. This resulted in a fatal accident.
Employee received crushing fatal injuries while assisting MSHA inspection of elevator inside of Preparation Plant.
Haul truck driver was going to pit to get loaded, traveled over berm, truck landed on cab in water
UNKNOWN AT TIME OF REPORT. A miner died when the front-end loader EE was operating went over a highwall and fell approximately 70 feet.
Operator was backing up close to the edge at a berm and was facing the opposite way. Operator was pulled backwards by the counter weight, operator attempted to turn the machine to stop it from rolling into the ditch.
The employee was performing routine preventative maintenance on a transformer when EE was fatally electrocuted.
A delivery driver employed by a water supply vendor was in route to the facility when the tanker truck overturned and slid off the road. Investigation ongoing
Operator was conducting remoting activities in an area known as the 2025 area when a large rock contacted the cab. Operator was found in the cab out past the designated no-go line and sustained fatal injuries.
It rained and the mining pit needed to be dewatered. EE was at the mine site alone. It was reported that EE's spouse, was in the truck on the mine site while EE went into the pit. EE moved the telehandler into the pit, parked it on the bottom of the pit parallel on the ramp. EE exited the tele. While standing outside of the tele facing east, the tele fell over onto EE, crushing EE.
EE was operating an excavator in Pit 2 at SPS Atascosa. Pit 2 is located between two tailings ponds. 9:18 AM one of the tailings ponds breached causing tailings mixture to flow into the work site. Tailings mixture tipped over the excavator EE was operating cab side down. The front window broke, tailings mixture engulfed the cab & 75% of the vehicle. Employee could not escape.
Victim was removing sand from the sand wall with a Volvo L120E loader and the wall sloughed off covering the loader and victim with sand.
The detonator had a dead battery. In order to set off the shot, employee brought up a loader. The loader battery was used to set the shot off. Employee was unable to be inside the shelter provided for blasting.
Employee was found entangled in conveyor 12 tail pulley. Responding authorities pronounced the employee deceased. Incident is still under investigation.
EE had been jack hammering to set a water trap for seals and traded off with coworker to take a break went over to rib sat down when rib roll 68"L x 24" W x 8" T came off at spad #3002/7483 on EE striking EE in the chest and abdomen area.
A miner received fatal injuries. After a bridge collapsed that the miner was operating a skid steer loader on. The miner was clearing refractory brick form a rotary kiln. The bridge that was installed by the property owner collapsed. The skid steer fell approximately 10 feet into a hopper, causing the payload of brick to engulf the operator causing fatal wounds.
Employee was operating a DM45 Highwall Drill on a drill bench. Employee was traveling parallel to the edge and the outside edge gave way. The drill slid off the bench and landed 25 feet below.
Miner did not follow protocol. Therefore causing the death of the same.
Employee in the thin veneer area was crushed by a pallet of stone that was placed on them by another employee in a wheel loader.
The employee was drilling a test hole in the surface pit floor to see if the rider coal seam was under the seam they were taking. Due to the freezing and thawing of the highwall a large chunk of rock approximately 4'x2'x3' came down striking the enclosed cab of the mobile drill, ripping the cab off and striking the employee.
An employee was crushed between a mobile bridge carrier (MBC) and the coal rib.
Trench was dug to repair drain pipe. No sloping or shoring protection system was in place. Trench was approximately 10 foot deep and 8 foot wide. Side wall collapsed and crushed worker.
A loader bucket on a CAT R3000H came down and crushed an employee while in use, resulting in a fatality. The injured employee was operating a UTV (gator) in Unit 43 near the feeder breaker (Stamler) when the incident occurred. This incident is still under investigation.
Employee died when they became entangled in the log washer.
At this time unknown dump point failure causing haul truck operated by the deceased to back and roll down the dump point.
Employee was standing on 172 shield and advancing 173 shield towards the face. The employee was found with an injury to the back of the head at 171 shield.
It was determined that the employee had previously mined the #3 entry on the 7S section. The employee had moved the CM outby 12 crosscut and had walked back to the last row of support in the #3 entry to adjust the tubing. A rock fell from just inby the last row of permanent support and struck the employee taking them to the ground. This fatality was investigated by MSHA.
Employee found face down on ground next to loader EE was operating. Responding authorities pronounced employee deceased. Incident is still under investigation.
