Service tech / Labor was helping a miner, The Tec was using a small step ladder to reach the work area. As they climbed the step ladder they hit their head on a piece of steel causing a laceration to the scalp. The Tec failed to use PPE at the time of the accident
Employee slipped while getting out of the Cat loader and slipped to the ground causing pain to left knee
Loader operator missed the first step when exiting EE's loader and fell to the ground. Operator fell app 6 foot. Operator received broken right forearm, laceration above right eye. Treated at Business Health and released
Employee was checking alternator belt, when a high gust of wind slammed the panel door out of EE's hand and struck EE in the back of the head. EE received a broken nose, laceration to left eye lid and a mild concussion.
Employee was getting into his loader and missed the last step and fell. He hit his chin on the upper step and cut his left hand on the door latch
On 10/7/14 I was contacted by ee and was advised that he had been doing mechanical repairs around the wash plant and felt pains in his left shoulder. EE refused medical attention at this time. On 10/20/14 ee contacted me and stated that he had set up dr apt for 10/20/14. On 10/21/14 and stated, that he needed to have shoulder surgery and it was scheduled for 10/29/14.
EE was driving a haul truck and complained of back pain. This took place on Sat.6-21-2014. I contacted ee on Monday reference the incident and told him to go to RediMed and get checked out. ee contacted me Monday at app. 1500 Hour and stated he will be off work until next office visit 6-26-2014
Reported to Irvings on 02-14-14 no medical action taken, On 4-24-14 MRI requested showed Rotator Cuff Sprain, No lost time Working, Pain Level higher Talked with Medical on 5-21-14 Requested time off. Surg on shoulder off work on 6-3-14 will be off app. 3 Months. Slipped on ice exit a loader
Employee was helping put a new starter in a loader. Stated that he was pulling on the starter and he had a sharp pain in his left shoulder
Employee was working in the scale house and walked out to the safe area and tripped on some rocks and fell. EE refused medical attention at the time of the incident. About three hours later EE asked to go into RediMed and have her hand check out. RediMed stated EE had fractured her Left wrist and it was going to be splinted. At this time she was able to return to work.
Working on ground by water pipe. Employee was stung by several bees. First report employee pulled a calf muscle and twisted his left ankle.
EE was using a step ladder to had oil to his loader. His foot slipped from the step and he fell from the ladder. He was app 3 foot from the ground when he slipped. He complained of pain and refused medical treatment. On Tues. 4-23-13 he requested medical for his knee and ankle. RedImed treated and put him on lite duty until 4-30-13.
Non-work back injury filed with workers comp. after the employee complained about pain in his lower back and left leg. Workers Comp. stated that it was work related due to the aggravation of his work.(equipment oper.)
EE reported he slipped, when he was working on a 4 foot step ladder. This was reported on 5-8-2012 but refused medical treatment. I was contacted on 5-9-2012 by EE and he requested medical treatment at RedIMed. EE complained of pain in his Right Elbow and forearm.
Operator reported he twisted his ankle but not sure if it was at work or home on 4-25-2012. On 5-2-2012 employee was put on lite duty By Doctor and was told to wear a boot cast to relieve the pain. Employee was moved from the work site to the office. No MISSED Work.
Employee had stepped across the frame of a conveyor when he slipped and twisted his left knee, causing great pain and swelling.
Employee was operating a cutting torch when the gas line burst causing a flash burn. He received 1 & 2nd degree burns to the face and neck.
Employee was setting up his portable crusher at Fairview Quarry #5. He was installing a cover over bottom conveyor. Cover slipped cutting his left ring finger.
Employee was helping replace a side panel on the crane, when he slipped off the top of the battery box and fell onto the track of the crane. He then rolled from the crane track and fell to the ground app. 4 feet.
The employee fell from app 8'. The employee was setting on a I-Beam and leaned out to push on a second I-Beam. The I-Beam slid causing the employee to fall from where he was setting. No safety equipment was being used at the time of the accident. ( Second time this was reported- no report no. on file )
Operator inattention, while operating equipment. Operator hit a concrete bunker for fuel storage tank. Operator received a laceration to forehead , received 5 stitches.
Employee was welding on a drag line chain and twisted his back, when he moved the chain. He was examined by three Physicians and was found to have two ruptured disks. Filed on works comp., hurt on duty
Employee was servicing a haul truck. When he went to remove the pan a safety bolt broke causing the pan to fall. He received a small Laceration to his head.

PULLING ON HANDLES TO OPEN BACK DOORS OF SEMI TRAILER (GENSET). ONE HANDLE POPPED UP AND STRUCK EE IN THE CHIN & KNOCKING HIM TO THE GROUND. RECEIVED SEVEN STITCHES & RETURNED TO WORK. WENT IN FOR SURGERY ON NECK 4/17/03 RELATING TO 3/21/03 INCIDENT.
