Complained of pain in right shoulder and upper arm. Subject was working on the crane.

Fall from ladders

EE was using a step ladder to had oil to his loader. His foot slipped from the step and he fell from the ladder. He was app 3 foot from the ground when he slipped. He complained of pain and refused medical treatment. On Tues. 4-23-13 he requested medical for his knee and ankle. RedImed treated and put him on lite duty until 4-30-13.
