Employee was cleaning catwalk and turned sending intense pain to EE's foot/ankle area making it hard to move and walk. Employee has had foot discomfort on left foot for a couple weeks and never told anyone. Employee was seen in ER, was given an injection for pain, and taken out of work for 1 day. EE was diagnosed with tendonitis.
Little finger on left hand got pinched between pry bar and beam.
Employee was lifting a steel plate. The plate slid on the concrete floor at which point the employee had their finger pinched in between the floor and plate.
Miner was attempting to loosen a bolt on a jaw crusher wedge bolt using a crescent wrench and having another miner strike it with a hammer. When the miner struck the wrench with the hammer, the wrench slipped off the nut and smashed the finger of the miner holding the wrench. Miner was wearing gloves. Left index finger fracture.
Employee was cutting an old section of conveyor belt. EE cut two fingers with a utility knife. Employee received stitches to ring finger. EE returned to work with restrictions. EE is scheduled for a follow up on Monday 7/22/24.
While driving loader, operator ran over a big stone that caused operator to be jolted inside the cab. While steadying self, operator placed left hand on the frame of the cab (where door hinges are). The door swung closed and pinned operator's thumb. Thumb was fractured, received 2 stitches, and given prescription antibiotics.
Haul truck hit a bump and the right glass window broke. Employee grabbed right armrest leaned to the left to avoid the glass, hand slipped off causing a small laceration to EE's right arm. Employee was taken to the hospital for evaluation. The wounds were cleaned & sutured. The employee was released from ER with No Work Restrictions.
while driving the haul truck over a recently constructed haul road the tires slipped off edge and went into a slight water puddle causing the truck to rock and the employee struck EE's head on an AC unit that was hanging from the ceiling. employee was taken out of work for one day and is now on restrictions due to lingering symptoms.
Employee arrived to work at normal time (6am) and performed normal duties (sitting in crusher shack operating jaw). Around 10 am employee starting having knee pain. Employee stated that EE does not know what caused the pain. Employee went to urgent care and was given prescription medication and taken out of work for 2 days.
Injured worker was team lifting a 60lb sheave when the sheave slipped out of coworker's hands and dropped on IW's right hand. When the sheave was dropped it pinched IW's right hand between the sheave and some steel guarding in the area that resulted in IW receiving 4 stitches to the right ring finger. Injured worker was placed on light duty for 2 weeks.
Unplanned fire on the property was observed at 8:30pm. Fire dpt was dispatched at 8:30pm. Site personnel confirmed dry tower area/belting was on fire at 9:15pm. Fire was extinguished by local fire dpt around 9:30pm. Hot work activities that occurred on the dry tower area around 7:30pm are believed to be a contributing factor. Last employee left the site around 7:53pm.
Employee came to work on 3/16 with right eye irritation, went to urgent care and was diagnosed with acute conjunctivitis. Irritation worsened; employee sought additional care and had a foreign body removed from EE's cornea on 3/18/23. Employee was given prescriptions and returned to work. Employee believes grinding activities on 3/15 caused the foreign body to enter EE's eye.
Employee was assisting with a screen change out during wet, rainy conditions during the shift. Employee went home, showered, went to bed, and woke up around 3am with pain in left eye and went to the ER. ER doc removed a small foreign body from EE's eye. Employee followed up with eye doc who returned EE to work.
Employee stepped on small piece of limestone and sprained left ankle.
While walking up a small pile at the blend 1 conveyor to change wipes, employee stepped on the loose material and left knee gave out.
The miner was driving a Bell B45E articulated dump truck up a ramp in the mine and hit a rut in the road causing EE to bounce up off the seat and hit EE's head on the safety handle located on the roof of the truck. The miner went to the local ER where EE was placed on light duty due to having concussion symptoms
Icy conditions on the ground at the approach of a set of steps leading to the platform. EE was looking at the ice while trying to use caution, when EE slipped in the ice falling to the ground and bumping head. EE first refused medical treatment. EE later decided to seek medical treatment, resulting in a diagnosis of a minor concussion.
Injured worker was bent over walking under a belt, stepped on uneven ground, rolled ankle, and almost fell. While trying to catch self from falling the worker strained lower back. The worker did not actually fall. The worker notified supervisor 2 days later, went to the ER where worker was given a prescription for a muscle relaxer and taken out of work for 5 days.
Employee tripped while exiting skid loader and caught thumb in door handle at the same time. Sprained thumb.
Holding a bolt head while another guy was using an impact gun to loosen the nut. The bolt shifted down enough to come loose and spun in my hand cutting though my leather glove and cutting my hand.
Plant operator was using Allied breaker to break large rock at Jaw Crusher. EE did not notice a bolt head at the top of the hammer was in contact with a part of the shielding around the jaw opening. As EE pushed the hammer head down upon the rock, the bolt head broke the plate weld loose on one end and the steel sprung against operator's right arm.
Employee stated that EE had decided to check the wedges on the side of the Jaw Crusher to make sure they were tight. EE did not get a lanyard to tie off, or a step ladder. EE said EE worked EE's way out on the side of the jaw, standing on the bottom web of the structure I-beam. EE slipped and fell four feet, primarily on ankle.
Miner was climbing the 4th step of haul truck with cooler over left shoulder and lost balance when cooler slid down shoulder. Miner fell, hitting left heel and left side on ground. Initial injury was a bruised foot/heel. Miner continued to feel pain in left side months after fall and returned to doc. Miner had surgery on 2/10/21 to repair a broken hip.
Employee descended steps to primary hopper and after turning around lost footing and hyperextended left leg.
Went to get into the Haul truck, stepped up on the foot board reached for the hand bar, hand bar had frost on it. EE's hand slipped causing EE to fall to the ground.
While throwing a rock into the jaw to help get a big rock out of the crusher employee's hand was smashed between the rock and the I beam in the front of the opening. This caused the skin to open on left thumb.
Employee was holding a hammer wrench (tool designed to be struck with a hammer) while second employee was striking the tool with a small sledge hammer. At one point, 2nd EE missed the wrench and struck EE twice on the back of the right hand; causing a small laceration and contusion. EE received 4 stitches.
EE was attempting to cut off a grease hose on a tailwheel section. EE stated the utility knife went through the hose much easier than expected and made a cut into left index finger. This work was being performed in the shop adjacent to the office.
Employee was assisting in reinstalling the top spring plate on a Deister screen. While doing so, employee's hand slipped off the plate as the plate was seated and got left pinky finger pinched between the plate gusset and screen gusset. Employee received 7 stitches to close the injury.
An aggregate laborer was attempting to loosen a cap on a fuel tank with a pipe wrench when felt pain in back. The laborer finished out the day doing normal duties and returned to work the next day and went about normal duties. At the end of the second day employee decided to seek medical treatment and was put on restricted duties for lower back pain.
While placing a screen panel on the catwalk another screen scraped EE's right forearm causing lacerations, requiring 19 stitches.
An employee was cutting skirt board rudder with a utility knife, when EE cut EE's left index finger with the utility knife, had to get five stitches
Employee was cutting cardboard with a straightedge and a razor knife. Knife slipped over straightedge and cut thumb of left hand.
While making a splice on Main belt, employee got finger lacerated while holding up on the Flexco bolt while nut was turned with an electric drill/driver. Employee was wearing heavy leather winter type gloves; removed glove and found finger lacerated.
Against procedure an employee attempted to track a conveyer while it was in motion buy accessing a ladder to reach the adjusters, While doing so he came in contact with the moving belt causing his left arm to be caught briefly. He received first aid for abrasions and contusions and has no restrictions. The employee was given a prescription for pain, making this reportable
The employee was walking toward the Jaw catwalk area when he rolled his ankle. X-rays revealed a broken bone in his right foot.
He parked loader and was trying to put window up from platform and he stepped back his right foot got stuck in the top step and he fell to ground. Right ankle fractured in 2 places and crushed heel.
Employee was loading a truck with a 980K loader and turned to look back, tweaking his back. This Employee has a pre-existing back injury which is non-work related.
Employee was adjusting the tightness setting on the Nordberg Portable crusher. He was reaching for a wrench with his left hand while resting his right on the hood. The hydraulic pressure released, pinching the tip of his right middle finger between lower adjusting nut and washer.
2 workers were welding, cutting and grinding on the screen deck to the wash tower and at lunch one worker's eyes started watering and burning.
While climbing down some steps and upon stepping onto a smooth steel platform, he slipped on some limestone dust and fell on his left side, breaking his left elbow. Employee also mess up his left shoulder, may have injured his lower back.
Employee was walking in the Jaw Genset trailer area and slipped on an icy spot, dislocating his left shoulder.
Employee was removing a guard on C-13 conveyor in order to change a return roller. He untied the guard and it swung down and struck him on the left foot. This resulted in a puncture wound to his left big toe.
Employee was removing bolts from the large cover for the blend chute, as he was taking the last bolt out it came down and hit his left thumb and cut it. He received 6 stitches.
Re-palletizing stone pinched right index finger when stone moved.
Employee was operating an excavator moving material at the waste pile and was stung by a bee on the right shoulder. He developed a rash on his chest and went to the ER and received prescription medication for this sting.
EE was helping another Emp. remove a rock from the portable crusher and while lifting on the rock, it shifted and pinched his thumb between the rock and the apron plate, causing an avulsion to his thumb. Emp. had surgery to repair avulsion to thumb.
Contractor was using a chain hoist through the eyebolt of 30 hp motor to position the motor into place. A mechanical failure caused the hook of the hoist to swing back into the face of the contractor. He was taken to the emergency room, where he was treated and released that evening. Contractor returned to his normal duties 2 days later.
EE was using a torch to help install gates on the pit feeder. He was holding the torch with left hand when the hoses moved and caused the torch head to swing around forcing the flame over the back of his right hand.
The wedges on the back of the portable crusher hopper vibrated out and the hopper sides opened up slightly. EE went to clear the rocks out of the way and after clearing the last rock, the back of the hopper fell down striking him on the top of the foot, fracturing it. The hydraulic cylinder which moves the hopper side up and down was defective.
