Miner was attempting to loosen a bolt on a jaw crusher wedge bolt using a crescent wrench and having another miner strike it with a hammer. When the miner struck the wrench with the hammer, the wrench slipped off the nut and smashed the finger of the miner holding the wrench. Miner was wearing gloves. Left index finger fracture.

Plant operator was using Allied breaker to break large rock at Jaw Crusher. EE did not notice a bolt head at the top of the hammer was in contact with a part of the shielding around the jaw opening. As EE pushed the hammer head down upon the rock, the bolt head broke the plate weld loose on one end and the steel sprung against operator's right arm.
Employee stated that EE had decided to check the wedges on the side of the Jaw Crusher to make sure they were tight. EE did not get a lanyard to tie off, or a step ladder. EE said EE worked EE's way out on the side of the jaw, standing on the bottom web of the structure I-beam. EE slipped and fell four feet, primarily on ankle.

Employee was holding a hammer wrench (tool designed to be struck with a hammer) while second employee was striking the tool with a small sledge hammer. At one point, 2nd EE missed the wrench and struck EE twice on the back of the right hand; causing a small laceration and contusion. EE received 4 stitches.

EE was attempting to cut off a grease hose on a tailwheel section. EE stated the utility knife went through the hose much easier than expected and made a cut into left index finger. This work was being performed in the shop adjacent to the office.

While making a splice on Main belt, employee got finger lacerated while holding up on the Flexco bolt while nut was turned with an electric drill/driver. Employee was wearing heavy leather winter type gloves; removed glove and found finger lacerated.
