While loading the power screen EE's thumb caught between the railroad tie and the power screen. This resulted in the separation of the tip of EE's thumb.

EE was changing screens on the power screen, lost EE's footing, fell backwards hitting the ground. EE has broken ribs, torn cartilage, and lacerations on EE's head.

The belt on the power screen was stuck. While trying to free the belt it came loose causing the fastener holding the belts together to fly free and cut employee's upper left leg.
