Showing all 13Employee missed a step while climbing off the back of EE's mechanic's truck. EE fell to the ground, hitting EE's elbow. X-rays showed EE fractured EE's elbow.
Employee entered feeder at wet plant to complete end of shift clean out, to manually unplug and remove materials build up. EE was shoveling in far corner of hopper when the material gave way. They lost footing and fell about one foot hurting their shoulder. EE stated that they regained themselves & started to climb out when a knee gave out and they fell again, about one foot.
Miner was assisting in attaching guards to conveyor. Used a pocket knife to cut away zip ties attached to conveyor and hand slipped, lacerating the finger of left hand. Transported to medical clinic and received five stitches. Returned to work the same day without work restrictions.
While sharpening a tool in a vice, in the process of reposition the tool employees hand got caught causing a laceration.
Employee lost their balance while opening the door to a skid steer loader. The incident resulted in a sprain to the employees left ankle.
Employee was lifting a screen, and felt a pain in his groin area. Employee didn't seek medical attention until March 25th. Upon a medical examination, a hernia was discovered.
While cleaning hopper out under grizzley on jaw feeder using a bar into hopper, bar hit back wall of hopper kicking bar off to the side striking me on the left check
While operating a scraper he hit a bump, causing a strain to his lower back.
While moving supplies, the Left palm of his hand was punctured by the end of tie wire.
EMPLOYEE WAS CHANGING SCREENS ON A CONE CRUSHER. AS HE THREW AN OLD SCREEN OFF THE SIDE, IT SNAGGED THE ZIPPER PULL ON HIS VEST AND PULLED HIM OFF THE SIDE OF THE DECK. HE WAS NOT WEARING HIS FALL PROTECTION. HE FELL APPROXIMATELY 13' TO THE GROUND. HE WAS DIAGNOSED WITH A BROKEN LEFT ARM AND A RUPTURED SPLEEN. HE WILL MISS 10-20 DAYS THEN RETURN WITH "NO LIFTING" RESTRICTION.
WHILE STEPPING UP ONTO A SCREEN DECK HE TWISTED HIS RIGHT KNEE.
FOUR EMPLOYEES WERE REMOVING AN ELECTRICAL SWITCHER FROM THE CONTROL VAN. WHILE IN THE PROCESS, THE BOX FELL TO THE FLOOR AND A CORNER LEG LANDED ON HIS FOOT. THE LEG ENTERED HIS BOOT JUST BEH IND HIS STEEL TOE PROTECTOR, STRIKING NEAR THE BASE OF HIS RIGHT BIG TOE. X-RAYS CONFIRMED A BREAK TO THE TOE.
