Showing all 8Removing brush from behind Olson Bins, stepped over a pile of brush, ground was further away then what was expected, foot hit solid ground and injured knee. Contributing factor was human error, not paying attention to footing. This is a pre-existing condition-happened at previous employer about a year ago, same leg and same knee.
Another miner was using a pry bar to close door on train car. IE was using a hammer to hit the lock on. The train car door sprung open and yanked the pry bar from the miner's hands hitting IE on the left side temple.
Cutting conveyor belt using a utility knife and cut left hand between thumb and forefinger. Miner received five (5) sutures.
Lifting drive shaft into place. Right hand palmar laceration.
Welding. Welding flash burns.
STEPPING OUT OF SKIDSTEER LOADER, MISSED STEP, LEG WENT BETWEEN SKIDSTEER AND BUCKET. LEG BECAME WEDGED, FELL TO LEFT, STRAINED KNEE.
