Fall onto or against objects

Miner began to ascend front-end loader ladder, grabbed both side ladder rails and went to step on first rung, foot slipped off rung and caught fall with arms jarring right shoulder.

Removing brush from behind Olson Bins, stepped over a pile of brush, ground was further away then what was expected, foot hit solid ground and injured knee. Contributing factor was human error, not paying attention to footing. This is a pre-existing condition-happened at previous employer about a year ago, same leg and same knee.

Another miner was using a pry bar to close door on train car. IE was using a hammer to hit the lock on. The train car door sprung open and yanked the pry bar from the miner's hands hitting IE on the left side temple.
