Showing all 49While using A-frame ladder to remove material from elevated beams, the RAM team member had already made several trips up/down the ladder. On way back up for another trip, ladder tipped over while EE was climbing, resulting in a fall. The RAM team member was taken to the hospital. Medical evaluation, including x-rays, revealed a fracture to both the tibia & fibula, requiring surgery
Metal plate had a burr causing a laceration to EE right hand.
The employee was changing skirting rubber. They stepped up on a piece of the framework to look in a hopper and when they stepped back down their foot landed on a piece of material and they twisted an ankle. It was initially treated as a sprain, but after not improving as quickly as expected, an x-ray was done on 4/22 and it was determined that there was a fracture.
The mine employee was using a 4' sleever bar and a 4 lb. hammer to remove mud that was built up under the Stamler drag chain. EE felt a pain in right shoulder. EE still had some pain later, so EE was taken to the clinic on 5/17/2024 and returned to work with no restrictions. On 7/26/2024 EE was seen by another doctor who evaluated the MRI and put EE on light duty.
Employee was building scaffold over conveyor belt. Employee slipped 18 inches catching armpit on the scaffold bar. This dislocated EE's shoulder. Shoulder was put back in place same day.
The mine employee was checking the buckets on a bucket elevator and got a particle of something in EE's right eye.
Miner drove a loader across feeder #1 when material had been removed. The plant operator had triggered feeder #1 and failed to communicate to the loader operator. The miner was injured as the wheels on the loader dropped into the feeder, causing miner to experience multiple body strains.
A haul truck driver with a history of asthma told the excavator operator that EE was having an asthma attack.
The employee was fabricating a base for a motor and gearbox using a steel plate 24"x24"x3/4"thick. It was on a steel work table in the shop. While repositioning the plate the employee smashed the end of left index finger between the steel plate and the table top. The finger was split open and required six stitches to close the wound.
While getting loaded the loader operator jarred the 777 haul truck that the employee was operating straining their neck and back. Employee's official notification date is in question. The official notification was given on 5/26/2021
Driving over bumps near the face causing EE to bounce around in the cab of the haul truck. Note: Initial treatment did not involve reportable treatments (restricted duty or prescriptions). Employee was released to full duty. On a follow up visit on 8/13/2019 employee was given prescriptions and was recommended therapy
Employee was retrieving model number on a gear box. In order to do this they used a manlift to elevate 24'. EE removed the guarding, retrieved information, and proceeded to remove the motor assembly. The motor was not properly secured, it fell striking the employees leg. EMS was called to the site employee was taken to DVMC for treatment. Employee was released on 6/13/2019
NOT WORK RELATED. Just after EE got to work EE began feeling chest pains. EE notified the safety coordinator. EE was transported to the Office to await emergency medical service. EMT's arrived the pain had decreased and the onsite EKG showed normal stat's. EE was transported to the hospital under the advice of the EMT's.
Employee was climbing up dozer to fuel the machine, as ee grabbed the grab bar ee right hand, ee left foot slipped causing ee to strain left shoulder.
While replacing the shaft guard on a D10 ripper laying on a work bench the injured felt a back strain.
At 5:17 during the work place exam, an employee discovered a body at the clarifier, an unknown individual that came into the mine site and committed suicide according to metro police investigators.
Employee went to kneel down to stab bolt in crossover belt guard and knelt down on J-bolt that was in screen.
Employee was climbing down ladder and felt pain in right knee.
The ee was climbing off the cone. His foot slipped between the ladder and cone. He fell hitting his right side on the top of the ladder.
Employee stepped down from a pedestal to a platform when he stumbled and tripped and fell towards the midrail of the handrail causing injury to forehead and nose. Employee received stitches on the forehead and had a bone fracture of the nose.
Crew was setting portable jaw, when block gave way and jaw tipped onto bobcat pinning EE inside cab.
EE WAS CLIMBING DOWN FROM LOADER AND FOOT SLIPPED OFF STEP. HE FELL BACKWARDS AND LANDED ON HIS BACK AND HIT HIS HEAD.
Was holding slack out of chain with loader lifted belt chain; twisted and caught finger between the chain and the belt.
Went into the screen to clear a jam in the rock chute. Was getting out of the machine onto ladder, foot stuck and pivoted foot resulting in a strain/sprain to left ankle.
Setting screen box, crane raised. While raised employee reached up to check the load to twist it, and pinched his right thumb.
Employee was cutting skirt rubber when the knife slipped and went through right finger.

Crossing over drill, slipped, caught self with right arm (pulling shoulder)
Slipped off equipment in the rain.
EMPLOYEE WAS LOWERING THE LINER INTO THE CHUTE WITH THE CRANE, THE LINER WAS A LITTLE TOO WIDE, EMPLOYEE STOPPED THE CRANE OPERATOR & WAS TRIMMING THE LINER. THE LINER SLIPPED DOWN THE CHUTE & THE UPPER END CAME UP PINNING THE EMPLOYEE.
Employee was changing a tool bar on a D9N dozer and hurt his right shoulder; told his supervisor his arm hurt, not that it was from the job. Never stating anything else, he then went to the doctor on 4/21/04. This is when he states it wasn't getting any better. Safety department received the paperwork from the job site on 4/29/04 to file this MSHA report.

WHILE ATTEMPTING TO ROTATE A MOTOR SHAFT WITH A PIPE WRENCH. THE SHAFT FREE WHEELED, CATCHING HIS HAND BETWEEN THE WRENCH AND HOPPE FRAME. LACERATION RT. INDEX FINGER REQUIRING TWO STITCHES.
EE WAS DISMOUNTING FROM CRAN3 WHEN HE SLIPPED APPROX 3', FALLING AND FRACTURING LEFT RIB.
EMPLOYEE STATES THAT HE WENT TO STAND UP, AFTER WALKING UNDER CONVEYER HE STOOD UP TOO SOON AND HIT HIS HEAD ON THE CONVEYER, HE WAS WEARING HISHARD HAT.
EMPLOYEE STATES THAT WHILE WORKING ON SCREEN UP WITH 17" IMPACT WRENCH 3/4" WOBBLE SOCKET 1/2" NUT. THE SOCKET SLIPPED OFF NUT, HITTING HIS R. EYE CAUSING ABRASION TO EYE.
WHILE INSTALLING A NEW GEAR BOX EE WAS HAMMERING WHILE A CHIVE 2 FT ABOVE HIM ON THE FRAME. HIS LEFT ARM WAS ON TOP OF THE GEAR BOX WHILE HE WAS TIGHTENING THE BOLTS WITH HIS RIGHT HAND. CHIVE FELL ON LEFT WRIST.
WHILE CHECKING K2001 TRACKING, THE EE STOOD ON THE SW SIDE OF THE BELT. HE REACHED WITH HIS LEFT HAND IN BETWEEN TWO ROLLERS TO GET A ROCK WHILE THE BELT WAS RUNNING CAUSING L ARM TO FRACTURE.
EE STATES WHILE SHOVELING UNDER VSI HE PULLED MUSCLES
EMPLOYEE STATES THAT WHILE ROLLING A BIG ROCK, HE SLIPPED IN SAND, STRAINING HIS BACK.
EMPLOYEE WAS GRINDING OUT DENTS IN A HOOD OF A TRUCK WHEN THE GRINDING WHEEL CAME LOOSE AND HITHIS LEG CAUSING A CUT WITH PIECES OF THE DISC EMBEDDED IN HIS LEG.
WHILE CUTTING BELTING THE UTILITY KNIFE SLIPPED AND CUT EMPLOYEE'S LEFT HAND NEAR THE THUMB.
EE STATES HE WAS CLIMBING ON A STOPPED BELT TO REACH A GREASE FITTING AND CUT HIS LEFT LEG BELOW THE KNEE.
EE PICKED UP RAILROAD TIES TO SET ON FORKLIFT WHEN ONE SLIPPED AND SMASHED HIS FINGER.
EE STATES HE WAS CLIMBING DOWN FROM A LOADER AND SLIPPED JAMMING HIS LEFT KNEE.
