I,NAME WAS DRIVING A 50 TON HAUL TRUCK #2927. WHEN HE DROVE INTO THE STOCKPILE AREA, HE LOST THE BRAKES ON THE TRUCK. (HE TOLD THIS TO HIS FOREMAN AFTERWARDS) HE HEADED THE TRUCK INTO A BERM. HT THEN JUMPED FROM THE TRUCK.
