Struck against a moving object

Haul truck driver ran over the berm where the haul truck went over the side of berm and landed on the passenger side of the haul truck. Miner made their way out of seatbelt and was out of the haul truck. Ambulance was called and was taken to the hospital where miner was released for minor injuries.

Fall from machine

Miner fell off a conveyer when EE was fixing the gearbox EE was not harnessed or tied down. EE landed on EE's back the wind was knocked out of EE. EE's co-workers helped EE up and proceeded to take to one of the co-workers vehicle and was taken to hospital. The hospital diagnosed with compressed spine and fluid on spine as of 1:30 pm on 5/24/2021 no further medical updates.

Struck against a moving object

Maintenance Mechanic/lube truck driver was leaving the laydown/boneyard in EE's personal vehicle and over traveled an old unused ramp (25' wide 35' at an average).

Injured Employee pulled up to pile to be loaded with material. Loader Operator loaded Injured Employee's first trailer, then pup trailer. Injured Employee proceeded to pull away when something in the transmission broke and gear shift snapped, hitting Injured Employee in the upper leg and knee area leaving a knot and bruising.

Over-exertion in lifting objects

Shoveling stemming material into blast holes and hurt wrist.

Miner was cleaning mud off of roller with a hammer when the belt and roller caught the hammer pulling arm between the belt and roller. The miner received cuts and abrasions on right arm. The miner was taken to ER for treatment and received a tetanus shot.

Tracking rock feed/conveyor when rock fell and smashed right index finger.

EE was using a torch to cut a piece of metal off on the load out hopper. And a hot piece of metal rolled down and into EE's left hand. EE got burned and a laceration on EE's left hand. EE was taken to Urgent care.

Accident type, without injuries

Operator was hauling from the active mining area to the crusher stockpile. Operator was approaching a 70 degree turn - unloaded. The truck was coming down a grade of approximately 14%. He began to apply the brakes, and they failed to engage. He made the decision to proceed straight down the hill to the berm area, and he turned the truck to the left into the berm

Fall to the walkway or working surface

EE was cleaning under the Pan Feeder at the Screen Plant. At break time he left to walk up to control room. As he walked down the 2 steps in this area, he stepped onto the expanded metal walkway. Walkway had small depression & he stepped in it, resulting in him twisting his ankle & falling down on walkway. Xrays were negative. Dx was ankle sprain & foot sprain.

Struck against stationary object

EMPLOYEE WAS DRIVING THE COMPANY 3/4 TON PICK UP FROM SHOP AREA TO THE R07 MINE AREA. ON THE 1ST CURVE OF THE MINE HAUL ROAD, HE LOST CONTROL OF THE TRUCK. THE TRUCK SLID SIDEWAYS & HIT THE BERM. THE TRUCK THEN ROLLED SEVERAL TIMES & THE EE WAS THROWN FROM THE VEHICLE. EE WALKED AWAY BUT WAS TAKEN BY AMBULANCE TO THE HOSPITAL.

Fall from scaffolds, walkways, platforms

EE SHUT DOWN THE GENERATOR FOR THE END OF THE SHIFT. AS HE WALKED OUT OF THE GENERATOR TRAILER, HE STEPPED ON TO THE PLATFORM AT THE TOP OF THE STAIRS. THE PLATFORM WAS COVERED WITH ABOUT 1""OF SNOW. AS EE STEPPED ON THE SOLID METAL SECTION OF THE PLATFORM, HIS FEET WENT OUT FROM UNDER HIM AND HE FELL DOWN, STRUCK HIS ARM WHEN HE LANDED AND THEN HE SLID DOWN THE STAIRS.

EE STATES THAT WHILE LIFTING A HEAVY WRENCH HE NOTICED SOME STIFFNESS & SORENESS DEVELOPING IN HIS LOWER BACK.
