Employee was performing regular maint. on boiler tubes. Employee alleges that the rebar he was using became lodged between two tubes. When he pulled rebar out he felt a pain in left shoulder. Employee started physical therapy as of 11/29/2007.

Employee was installing roll on raw mill. Roll hung up & using protapowers to get it to fall into place. Got hand into a pinch point when roll fell into place. Hand caught between a board and the 2 ton roll, pinching left index finger.

Employee was using a cutting torch to remove a beam in sections. When he turned with beam section in hand his left knee popped. This was not a reportable injury until employee was evaluated on 6-6-05 and the diagnosis was received today.

Employee was removing a light fixture as doing so dust fell from top of the pillar and covered the employees hard hat, safety glasses and clothing. Employee states that he did wipe off the top of the fixture before attempting to remove.

Employee uses airlance to blow dirt from boilers. Also uses rebar to break caked on dirt. He states there is a lot of hand, wrist & shoulder movement to this task. This task is required every day. Employee was given medication first. Then Dr. decided he needed thearpy. That was done on 10-12-04. Reported as soon as it became reportable.

Employee had been working on top of So. Boiler. Plant was getting ready to put boiler back on. As he was coming down the ladder he missed the last rung. Employee states that he felt himself start to fall and grabbed the ladder. By doing this he felt a pain in his back. Did not request medical treatment until 5/4/04.

Employee was changing brass on the rolls on # 3 Kiln. He picked up a 1 1/2" impact to tighten the bolts. Felt a pain in shoulder. EMPLOYEE DID NOT REQUEST MEDICAL ATTNETION UNTIL 5/5/04. AT THAT TIME HE WAS SENT IMMEDIATELY TO THE COMPANY DOCTOR.

EE stated that a rock was laying in front of steps. He kicked it out of the way, it was observed by the EE that the rock was about 2"X 2" & weighted less that a pound. This was a statement made by EE to his foreman the following morning when he reported his accident/injury.

EE WAS USING A JACK HAMMER ONT UNNEL FLOOR AND SHOVELING DEBRIS INTO BUCKET. HE WAS THEN DUMPING THE BUCKET OF DEBRIS ONTO THE TUNNEL BELT. AS HE WAS DUMPING THE BUCKET OF DEBRIS HE FELT A POP BETWEEN HIS SHOULDER BLADES. HIS NECK AND RIGHT ARM THEN BECAME PAINFUL TO MOVE.
