EMPLOYEE THOUGHT RAILCAR HAD MANUAL BRAKES LIKE MOST OF OUR CARS.THIS ONE HAD A TRIP SWITCH EMPLOYEE TRIED TO LOOSEN BRAKE LIKE A MANUAL CAR AND INJURED NECK.EMPLOYEE FIRST THOUGHT ARM HAD BEE N INJURED WHICH TURNED OUT TO BE NECK INJURY.
EMPLOYEE WAS CLEA RING SMALL BUSHES OFF OF SETTING POND DAM FACE WITH A CHAIN SAW CHAIN SAW UNEXPECTEDLY KICKED BACK WHILE IN USE & ACROSS LEFT KNEE AREA OF EMPLOYEE
MAINTENANCE WAS WORKING ON ENCLOSURE AN ADDITION TO THE PUMP HOUSE. EMPLOYEE WAS ON TOP OF EXISTING ROOF REMOVING SECTION OF OLD TIN THAT WERE IN NEED OF BEING REPLACED. WHEN HE STEPPED ONTO A ROTTEN PIECE OF FRAMING, WHICH BROKE UNDER HIS WEIGHT. EMPLOYEE FELL DOWN BETWEEN THE RAFTERS CATCHING HIMSELF IWTH HIS LEFT ARM. HIS LEFT SIDE HIT ON THE RAFTER FRACTURING 3 RIBS.
EMPL CLEANING DUST COLLECTOR DISCHARGE HOPPER THROUGH PORTHOLE.WELD BROKE ON SCREEN GUARD CAUSING IT TO FALL INTO ROTARY FEEDER PINNING EMPL RIGHT HAND BETWEEN SCREEN GUARD AND SIDE OF WALL.
MAINTENANCE PERSONNEL WERE CHANGING OUT ONE OF THE JAWS ON THE CRUSHER. FRONT END LOADER HAD BEEN USED TO BRING CUT NEW JAW AND HAD DUMPED IT ON THE GROUND NEAR THE CRUSHER. EMPLOYEE HAD HOOKE D A PULLING BAR INTO THE JAW AND WAS PULLING IT ALONG ON THE GROUND, WHEN HE FELT A STING IN HIS BACK. HE CONTINUED HIS DUTIES AND FINISHED OUT THE DAY.
EMPLOYEES FINGER GOT CAUGHT BETWEEN AIR SLIDE AND SHARP EDGE OF GUARD WHILE REMOVING AIR CAUSING FINGER TO BE CUT.
CARLESSNESS ON THE PART OF THE CUSTOMER TRUCK DRIVER, BY NOT CHECKING TO SEE IF ALL LOADING HAD BEEN STOPPED OR FINISHED ON HIS TRUCK BEFORE MOVING. NEGLIGENCE ON THE PART OF BOTH THE SUPERVIS OR AND HOURLY EMPLOYEE FOR NOT CHOCKING THE TRUCK BEFORE LOADING HAD BEGUN. BOTH WERE AWARE THAT IT WAS A COMPANY SAFETY RULE THAT NO VESSAL HAS TO BE LOADED BEFORE CHOCKS HAD BEEN PLACED TO
EMPLOYEE WAS PUSHING A WHEELBAROW LOADED WITH OLIVINE AGGRAGATE OUT TO DUMP. WHEN HE STARTED TO DUMP THE MATERIAL, THE SUDDEN SHIFTING OF MATERIAL JERKED HIS ARMS FORWARD, CAUSING HIM TO STRAI N OLD INJURY.
EMPLOYEE WAS AT7EMPTING TO CLOSE DOOR ON RAILCAR WHEN PRY BAR SLIPPED OUT AND FELL BACK ON THE GROUND AND SPRAINED HIS ANKLE.
REPLACING ACCEL9RATOR LINKAGE WHILE MOTOR WAS STILL RUNNING AND WHEN ENGINE ACCELERATED HE JERKED HAND BACK RESULTING IN CUTTING RIGHT INDEX FINGER AT KNUCKLE.
