Audiological review due to STS. This employee experienced a recordable degree Standard Threshold shift in the Right ear on the 2023 annual review when compared to the 2006 baseline. The STS was confirmed on subsequent testing. Review of the conservation hearing program records indicates the STS does meet the requirements for recording based on the information provided.
An employee was walking to the mobile shop from the parked loader, when EE slid on some mud and fell. As EE tried to get out of the mud, EE's boots became stuck into the mud. Employee lifted right leg, felt a pop in the right knee, and fell to the ground.

Employee was loading rail cars and walking on/near the tracks. EE felt a poke in the foot and discovered a piece of wire had punctured the sole of EE's boot and entered EE's foot.

Lab tech lifted bucket (approx. 20 lbs.) about 36 inches over a rail to dump into a dumpster felt a pop. EE refused medical care at that time, iced and monitored it. Next day EE stated it felt better. On 10-26-21 EE stated it wasn't getting better and wanted to see a Dr. EE was given restriction and Tylenol. On 11-24-21 MRI was performed. Surgery was scheduled for 12-16-21.

Employee was cleaning rail cars. Employee came to a door that was difficult to open and pushed on the bar employee was using harder. The door freed up and the employee hit the ground with left shoulder.
Employee pushed open a door to go to the locker room at the end of the shift. The kick plate on the door pulled loose and got wedged in the door way. The employee walked through the door and tripped over the kick plate stuck in the door way.
Employee was cleaning out an elevator when EE's elbow began to hurt.

EE was cleaning with a loader and dug a hole in the ground. EE then backed into it and injured back.
EE was filling windshield wash reservoir and it squirted into right eye. Employee was wearing safety glasses. The fluid came up and under the bottom of the glasses.
Employee walked into the maint shop with muddy shoes and slipped and hurt knee. Could not walk on it after it happened. EE was seen by a Dr. and possible torn ligaments.

Employee was using a pry bar as a hammer and felt their shoulder pop, but said they were fine. Their shoulder started hurting worse on 8-16-16 so they were taken to the dr for evaluation. Dr put employee on restrictions until seen by orthopedic dr.

Pregnant employee missed a step and fell hitting the top of her belly. She was taken into the ER just as a precaution because she was pregnant and they decided to keep her overnight because she is a high risk pregnancy. She was released the next day.

EE was barring out pit and felt pain on lower back. We scheduled EE appointment at clinic and clinic started him on physical therapy with no restrictions. Thus constituting as an MSHA reportable pertaining to physical therapy.
EE was changing heater hose on a loader while doing so he hit his hand on a hose clamp and cut his hand. The EE received 2 stitches on the top of his hand. This constituting as an MSHA reportable accident.

EE was going down to the car puller to unhook the winch and while doing so he rolled his ankle. EE went to the ER and they put him in an air cast and no restrictions, this constituting in an MSHA reportable accident.
EE was re-stacking pallets with broken bags on them. EE felt pain in lower wrist. Doctor placed EE on restricted duty of no lifting with left hand for 5 days, thus constituting MSHA reportability.

EE was throwing 100 pound bags. Felt pain in wrist at the end of shift. Went to doctor on 11/30 and was told EE had tendonitis. Restrictions of not lifting anything over 10 pounds was put on EE resulting in MSHA reportablity.
Employee was removing a guard to work on a return spool for a railcar mover, when he felt a twinge in his lower back.
Employee was using a sledge hammer to clean out chunks off the dryer. Employee hit side of chunk and hit back wall of dryer hurting his left shoulder. Employee was put on restrictive duty, constituting MSHA reportable.
Employee was unplugging the coal pulverizer when he got burned by a blast of fire - burned his arm and lips. The burns were deemed 1st degree and employee was given prescription ointment, constituting MSHA reportable.
A Loader Operator was spreading dried material on a slick ramp and the wind was blowing N.W. 5 to 15 mph. The wind blew back the material on the windshield of the loader, the operator could not see the truck coming up on the right side and turned to the right tearing off the cab of the truck.

EE was stretch wrapping a pallet of 50# bags of bentonite, when he stepped beween the forklift stop and the moving stretch wrapper. The pallet that the bags were on, pinned his left leg between the forklift stop and the pallet of bags, causing bruising of the leg and light duty work.
EE was throwing 100# bag when he felt some pain in left forearm. Told his leadman who in turn reported it to the plant supervisor.

EE using jackhammer to bar at hammer mill. Turned to move hammer to left side of mill door and got his left pinky finger knuckle pinched between bottom left of the door edge and jackhammer.
EE was using 1/2""drill, drilling out fluid bed dryer grid. Drill jammed, causing the drill and EE's R wrist to continue in a clockwise motion. Twisting caused wrist to rapidly go into strained position, causing wrist strain. Wrist was immobilized with a splint, thus constituting MSHA reportability.
EE hit chin on bulk bag machine support arm when bag strap fell off, causing arm to move upward. Received stiched to cut.

Employees were baring out coal chute to Rotary Dryer when they started to fell dizzy and light headed. They noticed a smell when entering room but thought it was nothing. While stepping off working platform at coal pulverizing feeder, EE missed top rung with foot sudden drop and catching himself from falling-jarred lower back. He missed 2 days of work and sought chiropractic care thus constituting MSHA reportability. MIssed 5/12 and 5/13, not scheduled 5/14, returned 5/15 w/o restrictions
Ee was unhooking wench cable from hopper railcar so it could be pulled back. While attempting to remove hook from car by using hands to pull on it, hook and ee hand hit him in the mouth, chipping tooth
While throwing 100 lb. bags of material, left wrist started to hurt and began to swell. Diagnosed as wrist strain. He received temp. usage restriction to graduate to 100% usage in 2 weeks (restricted duty). Did not go to the doctor until 3-6-07, that is diagnosis date.
dusty and noisy 1/17/07: called Rick to obtain more information. EE was running a hoist, shive 056got bound, because of noise & dust, EE could not hear radio and the cable struck his thumb.

Employee's right foot was run over by right rear tire of forklift. Operator did not see him sweeping the floor behind him while backing up. Bruised foot.

EE cleaning trough roller on 2A belt conveyor, when right index finger was caught between roller and syntron pan feeder on 4/22/2006. On 5/2/06, EE decided to seek medical attn. X-ray taken on 5/2/06 revealed finger tip fracture, thus constituting MSHA rptblty on that date. No work restrictions ordered, no lost time.

Ee was exting bobcat skidsteer loader and didn't notice support beam for granular bulk truck load out canopy. He hit head on support beam and wrenched neck. He was examined with a negative x ray, but a smucle relaxer was preseribed and 1 shift of work was missed. Physician ordered 1 shift to be missed-that he not return that next evening.
EE was barring hammermill from breaker plate door and hammers were not done spinning. Bar made contact with hammers smashing right thumb between bar and hammermill housing wall. EE required 3 stitches.
EE slipped while dismounting loader - pulled/strained left shoulder. Lifting restriction.
Ee accident/diagnosis as Muscle Strain on 7-15-05. 12-27-05 ee noticed lump in area of strain and sought medical opinion 2 dr . 1 st did not, but 2nd did diagnose it as a hernia on 1-3-06. he returned to work that afternoon and hernia surgery was scheduled for friday 1-6-06.
EE slipped on expanded metal walking surface because of outside weather conditions being foggy and cold, made walking surface extremely slippery. Fell backwards on back and neck. Lost time reportability.

EE STEPPED ON CHUNK OF BENTONITE AND TWISTED KNEE. EE WAS STARTING UP DRYER. HE WENT TO UNLOCK TOOL BOX AND SIREN SOUNDED. HE WAS RUNNING TO CONTROL ROOM AND A CHUNK OF BENTONITE FELL FROM THE INCLINE BELT, HE STEPPED ON IT AND TWISTED HIS RT. KNEE.
LOADER OPERATOR SHUT DOWN PRIMARY SYSTEM TO CLEAN HAMMERMILL BREAKER PLATE AREA. THIS ACTION WILL AUTOMATICALLY SHUT OFF 2A BELT. HE DID NOT SHUT OFF THE FIRES TO THE VIBRATING PAN FEEDER WHIC H ARE LOCATED DIRECTLY ABOVE 2A BELT IN THE CRUDE PIT AREA. BECAUSE THE CLEANING TOOK LONGER THAN ANTICIPATED THE HEAT GENERATED FROM THE PAN FIRES IGNITED 2A BELT & THE SLIDER BED.
