While operating a loader underground EE stated they hurt their lower back due to the uneven ground in the area where they were loading material. The EE was taken to the clinic where they were diagnosed with a lumbosacral strain in their lower back.

While employee was attempting to open a pipe gate, middle finger on right hand got caught in between a cross piece and the lock on the gate. When the EE tried to pull their finger loose, they ended up pulling off the tip of their finger.

Miner was stepping down the ladder rungs from approximately a 30" platform when miner's glove hung up on a chain hook. This caused the miner to lose balance and fall to the ground injuring ankle.

Miner was changing engine air filters on Caterpillar 769C haul truck. When EE pulled the filters out of the housing, EE twisted at the waist to set them down on the deck of the truck. The miner felt a shooting pain in lower back.

Miner was dismounting bucket on the powder truck that was lowered to the ground when miner caught boot on the edge of the bucket. Miner lost balance and fell to the ground. Miner sustained injuries to elbow and back. Miner did not immediately request medical attention. At approximately 1:45 that day miner requested to seek medical attention.

Miner was exiting Cannon bolting machine frontwards. Miner slipped on the top step and fell about 4 feet to the ground, landing on feet with legs straight. With legs straight they did not forgive the landing and the employee's back was injured.

Miner was tightening a nut with a hand wrench and had a pain in EE's bicep. EE stepped away from the machine and got lightheaded and fell forward to the ground. EE was with two miners that helped EE up and to the breakroom where EE awaited the manager to take EE to be evaluated by medical personnel.

Miner was walking from mill building to Line #1 loadout. While walking, miner tripped and fell forward over the rail road tracks. Miner was not able to catch self with hands to lessen the impact and miner's face struck the ground. This caused injury to mouth, chin, and left knee. Miner's lip was lacerated and required sutures.

The miner was walking through shop prior to shift, to put lunch in the refrigerator. The shop lights had not been turned on, yet. As EE rounded the work bench, EE tripped over an air brake pod that had been stored under the table for future use. The miner was not able to catch self with hands and fell onto the right arm/side and hit face on the floor.

Miner was on top of TAMKO bin shoveling 1 1/4"X 1/2" aggregate that had overflowed onto the bin top. When going into the rock pile with shovel, the miner felt a sharp pain in elbow.

Miner unloading over-loaded trailer, was entering trailer second time when miner realized trailer was moving away from dock, Forklift was 1/2 in 1/2 out, and slid back and fell against dock plate then at angle to ground, when trailer stopped leaving forklift at an angle, with rear counter-weight end on ground, forks in trailer. Miner dismounted equipment.

Miner was operating a CAT D-350 Articulated Haul Truck, hauling C-4 Dust to Stockpile. Upon returning in Snowstorm to Line 2 Load-out, Right Front Drive wheel slipped off roadway into a drain depression, jarring the driver in cab and causing later back pain.

Miner (Maintenance - M5) was with Manager of Fine Grind/Mill Operations and was attempting to cut thru a piece of conveyor belting. Miner had noticed his work utility knife had a dull blade, and upon changing it with new one, was cutting belt when knife skipped/slipped and cut his right lower leg. Miner was transported to OCCU-MED and 4 sutures were used to close small laceration.

Miner is a Mill operator and was operating a C4S Packing machine. Miner was palletizing 50 lb. bags of C4S material when he felt a sharp pain suddenly in his left forearm area. Miner was taken for medical evaluation, and placed on lifting restrictions until a scheduled MRI X-ray can be conducted. Miner is working w/restrictions.

Miner operates a WDM Drill, noticed drill bit was dulling, went to Drill Pad area, to change out bit. Miner struck old bit w/2 lb hammer due to being stuck, metal hammer wasn't strong enough and a piece broke off, imbedding itself in upper G.I. area of miner, with downward angle. Piece had to be surgically removed by Specialist/Surgeon. Surgery successful on removal.

As stated by miner, shoulder discomfort has occurred to both shoulders, over a reported period of 10 years. Issue began gradual, but with worsening discomfort, over the last six years. This is the first time miner/powderman has officially sought diagnosis/treatment for reported issues. States after loading production holes for explosive detonation, shoulder hurts at end of day!

M-5 Maint. Mechanic/miner was raising over-head garage doors to open shop. In process, miner felt pain along right "Ribcage" area. Miner thought he'd "Pulled" a muscle. Did not seek immediate treatment. Treated on 10/14/11.

Notification Report Only: Miner suffered a severe heart attack unrelated to job duties, while @ work. Miner was transported from site to Hospital in Joplin, MO. with what has been determined to be coronary blockage. Miner received a "Stint" following ballon surgery, and may require further procedures.

Miner/Loader Operator was dismounting a 1996 CAT 970 to pick up a cone, when his right leg banged against the loader platform, scraping his shin which he grabbed, losing his safety 3-point contact and causing him to fall from ladder steps and injuring his pelvis/hip. Miner suffered fracture to his pelvis.

Miner dismounted a skid-steer loader to retrieve a shovel from near a "Trough" auger, miner ducked under the elevated auger frame going to get the shovel, but forgot to duck going back to his skid-steer, striking his hardhat on the auger bin. Miner suffered a mild stain to his neck. Miner returned to work w/restrictions.

Miner/Mant. shop, was climbing down from elevated position on gradall scaler, when he slipped, losing balance, sliding off fender skirt, approx. 4.5' to floor, bracing w/hands. Position was narrow, without elements of 3-point contact, and too low for fall protection device/rig. Inj. recordable because of wrist brace, miner rtn. to work.

Miner operating frontloader clearing drifts.While turning steering wheel, he felt a spontanous pain (like a pinched nerve) in L. shoulder joint. There was no collision, jostling, or disruption in cab when this feeling occured. Miner waited until next morning, when he said it was still sore, taken to Doctor. Work restriction for 1 week and NSAID meds. given, to be re-eval. 3/18.

Miner was repairing a discharge on a Tanker (semi-truck unit), and was using a "Cheater Bar" (steel pipe) to realign pipe to attach the lock clamp, when the miner felt a pain occurring in the middle section of his stomach region. Injury evaluated medically as a mild/mod. "Umbilical Hernia" w/o obstruction. Miner placed on lifting restrictions. Miner allowed to drive.

Maint. Miner was standing on elevated screen cover, (with handrails present/fall protection), when right foot slipped and miner fell against handrail/mid rail, twisting right leg/knee. Miner did not seek medical treatment @ time. Miner sought treatment for sore knee on 19th, and is working (with brace) awaiting later MRI. Knee diag. as strain-sprain.

Miner was positioning wooden blocks and sliding them (blocks), under the primary shell housing of the crusher. Miner had been working with other personnel, but decided to slide/spot the wooden block himself, straining his left shoulder. Miner did not seek immediate treatment, until mid-day (next date) 10/09/07 @ noon.

Miner was moving long sections of (approx. 21') steel piping from/between trailer and storage area. Miner thought this was how he was injured ? Miner had been instructed to wait for assisstance to arrive, but proceeded to move material on own. Accident/injury not reported until late Monday, Oct. 8th. Rolla Field office called, as well as (800) 746-1553.

Incident occurred 08/17/07 and was reported by miner, 08/20/07. Miner was assisting Mine Supt. in moving some large Mine fans to new positions for draft air, during which a fan was re-started and the miner inhaled small cloud of dust causing some lung irritation. No mask was worn, but was available. Medical, Breathing Treatments and shot. Miner has returned to full work.

Miner (Chief Electrician) was perfoming work on a conveyor (Weigh) belt, adding a load cell unit, when metal piece shifted slightly, causing an approx. 1" length laceration to the miner's index finger. Miner was transported to medical facility, where physician cleaned wound and applied (5) sutures. Miner released to full duty.

Miner stated that approximately 2 wks. prior, he had felt a slight pain in his L shoulder when trying to pull an actuator chain to open a flow valve to load-out. On the 16 th of July, the butterfly valve was again stuck, and required a extra hard tug to open, this time causing a possible sprain to the left shoulder muscles. Driver this time sought medical treatment.

Miner was climbing a external ladder @ the Line #2 load-out section to assist in cleaning out a blocked discharge chute following a rain, when he struck his head on a piece of angle iron support. Miner was wearing his hard hat, but felt a jamming sensation in his neck. Miner did not seek medical evaluation until the next day.

Miner was operating a front loader near the face area of M/NM Limestone mine, when while traveling/loading rock from floor, the equip. blade struck a high spot of solid rock on the floor, stopping suddenly. Miner strained back. Seat belt was in use, Miner did not seek medical care till 06/22/06.

Miner was assigned to maintenance group, and was called upon to assist in checking a railcar (covered hopper-BNSF RR), to see if assistance in closing an unloading grate under the car was possible. Miner injured swinging a hand sledge hammer (8 lbs.) against the grate sliding door jam. Strained lower back/leg. Miner did not seek treatment immediately.

Miner is assigned to maintenance, and was changing an alternator out of a gradeall scaling machine, miner had to lay against his own left arm in loosning the attachment bolts with wrench, apparently applied too much pressure to elbow region, and suffered a pinched nerve. Placed on restricted duty for approx. 5 days.

Miner was dis-mounting a ladder rung to step down to the floor @ the mill operations, when he stated his foot (L), slipped and he felt a "Popping" sensation followed by a stabbing pain. Miner was transported to company medical provider where an x-ray revealed a small fx above his left little toe. Miner was wearing safety boots.

Miner was attempting to activate the load-out chute @ the chip's bin, when upon pushing the manual lever forward to start loading, he stated the lever was "tight", and required more leverage to push. Miner stated he felt a slight "Pop" behind/below kneecap, and soreness developed in upper calf, (L), miner was evaluated and diagnoised with a strained calf.

While operating roller-mill equipment, EE was talking to coworker. EE stepped back & tripped over a pallet. EE stated he received a twisting type of discomfort to R-shoulder & R-hand. EE did not wish medical eval. at the time, but reported medical eval. request on 6-13-05. Sent to company doctor on 6/13/05 & placed on restricted duty.

Miner stated while approaching an intersection, driving a frontend loader, he traveled across some rough surface, jarring him while his neck was slightly turned, causing some discomfort. Incident was reported to supervisor, but miner didn't seek medical treatment. Pain continued and miner seen by company doctor on 6/22/05. Working now with restrictions.

Miner was assigned to haul backfill material into mine. While watching material being dumped in mirrors, driver said he prepared to stop his truck, when his feet engaged the brake and accelarator together. Haul truck lurched forwarded and struck pillar. Driver walked a distance to the maint. shop and when being driven to medical clinic for drug screen, stated his back was sore.

Miner was doing ground contol & roof scaling in the area of pillar 2P103 on the R&S section of the working face, when some loose rock fell near him causing him to step forward suddenly to avoid being struck. He lost his balance and fell forward off a muckpile, causing him to flip head over a couple of times, striking his head and causing a small bump and cut.

EE WAS EXAMINING A ROOM WITH FELLOW SCALER. WHILE LOOKING OVERHEAD AT ROOF CONDITIONS, HE STEPPED INTO A HOLE IN THE FLOOR. HE STRUCK HIS LEFT LEG ON A ROCK PROTRUDING UP FROM THE FLOOR OF ROOM. HE SUFFERED A LACERATION WHICH REQUIRED 22 STITCHES. RELEASED BACK TO WORK WITH RESTRICTIONS.

HE WAS WEARING HIS SEATBELT. NO DAMAGE WAS FOUND TO TRUCK. HE STATED HIS BACK HURT. HE ALSO STATED SPEED MIGHT BE A FACTOR.
