EMPLOYEE WAS STRIPPING INSULATION TAPE OFF OF A INSULATOR, THE KNIFE SLIPPED AND LACERATED HIS LEFT ARM.
CUTTING APEXWHEN HIS KNIFE SLIPPED AND CUT HIS RIGHT INDEX FINGER.
EE WAS CLOSING THE DOOR ON A DRILL WHEN THE HANDLE CAME OFF AND CRUSHED HIS THUMB.
EE WAS STEPPING OFF OF THE BOTTOM RUNG OF HER TRUCK. SHE STEPPED ON A SMALL ROCK WHICH ALLOWED HER ANKLE TO TURN OVER. SHE FELL TO THE GROUND.
A CHAIN HANGING FRON THE RIB SNAGGED THE RAILING ON THE TRUCK BEING OPERATED BY INJURED. IN AN ATTEMPT TO REMOVE THE CHAIN THE RAILING SLID AND CAUGHT HIS FINGERS.

WHILE CARRYING A PUMP, EE SLIPPED IN THE MUD, FELL AND STRUCK RT SIDE AND SHOULDER.
ROOF FELL ABOVE ROOF BOLTS.
CUT RT. INDEX FINGER WHILE CHANGING UTILITY KNIFE BLADE. FINGER TOOK SIX SUTURES.
REMOVING A BOLT FROM A BOLTER, BOLT BROKE & HANDSLIPPED & CUT FINGER
EE HIT HIMSELF IN THE CHIN W/A PRY BAR. THE BAR CUT HIS CHIN REQUIRING 9 STITCHES.
EMPLOYEE HAD RAISED A HYDRAULIC BOOM AND THEN ATTEMPTING TO BLEED THE LINE OF PRESSURE. THE BOOM FELL AND THE EMPLOYEE JUMPED AND HURT KNEE. SOUGHT TREATMENT ON 8-16-98 AND HAD SURGERY ON 8-25 -98.
EE WAS REMOVING UTILITY LINES WHEN THEY REACHED THE NEXT SECTION. THEY LOCKED OUT THE VALVE ABOVE THEM & THEN RETURNED TO DEPRESSURIZE THE LINE. THE VALVE WAS STICKING & THE EE TRIED TO FO RCE IT OPEN. HIS GLOVES WERE WET & HIS HAND SLIPPED OFF THE VALVE. AIR PRESSURE THEN CONTACTED HIS ARM
RETURNING TO BARROW AREA VIA RAMP. ACCELARATED DOWN RAMP AND LOST CONTROL. HIT HEAD ON ROOF OF SCRAPER. FRACTURED T-12 VERTEBRA. DOES NOT APPEARINDIVIDUAL WEARING PROVIDED SEAT BELT.
EE WAS UP ON A POLE PUTTING ALUMINUM WIRE INTO TRAVELERS, WHE HE REACHED OUT TO GRAB WIRE, EE SUFFERED A STRAIN TO RIGHT SHOULDER. EE WAS PLACED ON RESTRICTED DUTY FOR 3 DAYS.
PINCHED AND BETWEEN THE DRILL AND A WATERHOUSE.

HE DIDN'T CHECK THE AREA FOR SAFETY AND THUSLY DID NOT NOTICE THE MANWAY COVER ON TOP OF THE L1- 270/230 RAISE GATE (COVER) AHD BEEN LEFT OPEN. WHEN HE STEPPED BACKWORD SO HE COULD SEE HIS COMBINATION LOCK, HE STEPPED INTO THE OPEN HOLE, FELL 3' AND SUFFERED A BROKEN (R) INDEX FINGER.
EE WAS RESUPPLYING HIS GROUND SUPPORT WIERE. HE LEANED A PIECE UP AGAINST THE RIB AND TURNED TO WALK AWAY WHEN THE PIECE FLL AND STRUCK HIM ON THE NECK.
EE WAS WORKING ON AN ELECTRICAL NIP WHEN HIS SAFETY KNIFE SLIPPED AND HE CUT HIS FINGER.
EE STEPPED OFF A CROSS MEMBER OF A SIZZOR TRUCK TO THE GROUND (APPROX 2') WHEN HE CONTACTED THE GROUND HE STATES HE FELT SOMETING SNAP. AT THIS POINT HE FELL TO THE GROUND AND COULDN'T MOVE HIS LEG BELOW THE KNEE.

EE HAD PARKED THE TELEDYNE TRUCK NEXT TO TWO TAIL GATES FROM HAUL TRUCKS THAT HAD BEEN STACKED ON THE GROUND. AS HE BEGAN TO LOWER THE OUTRIGGER ON THE TELLEDYNE TRUCK, ONE OF THE TAIL GATES SLIPPED AND LAYED ON TOP OF HIS FOOT.
WENT TO TURN AND TWISTED RIGHT KNEE-POPPED LIGAMENTS. WENT TO DR. ON 11-27-97.
WHILE WALKING PAST THE COAL SILO, EMPLOYEE NOTICED HOT ASHES COMING FROM THE BOTTOM OF THE CHUTE. HE IMMEADIATELY NOTIFIED HIS SUPERVISOR. SIX HOURS WAS SPENT PUTTING THE COAL OUT. THIS WAS DU E TO THE PROCESS OF REMOVING THE REST OF THE COAL WITHOUT ALLOWING AIR TO CAUSE FURTHER IGNITION. THIS FIRE WAS OF SMOLDERING NATURE ONLY, THEREWERE NO FLAMES AND IS... REPAIR ON GASOLINE FUEL PUMP LINE FROM TANK ON WELDING RIG RESULTED IN GAS ON HAND & ARM OF JACKET. THE JACKET WAS REMOVED & HANDS WASHED SEVERAL TIMES BEFORE RETURNING TO ARC WELDING. HOWEVER ENOUGHT FUMES HAD PENETRATED THE SHIRT UNDERNEATH TO EASILY IGNITE FROM SPARK WHICH RESULTED IN 2ND & 3RD DEGREE BURNS ON RIGHT HAND & WRIST.
EMPLOYEE WAS POURING #2 FUSION FURNANCE WHEN A CRUCIBLKE FELL AND STRUCK HER LEFT FOOT ABOVE THE STEEL TOE. EMPLOYEE WAS SENT TO THE DOCTOR ON 11/09/97. I DID NOT RECEIVE THE C-4 REPORT UNTIL 11/24/97 STATING SHE HAS A HAIRLINE FRACTURE. THIS REPORT WAS DEVELOPED THE SAME DAY.
EMPLOYEE DID NOT REALIZE THAT HE HAD BEEN BITTEN BY A SPIDER. HE CAME TO ME ON 11/12/97 WITH HIS SYPTOMS AND I SENT HIM TO THE DOCTOR. THE DOC SAID HE HAD BEEN BITTEN, HENCE THIS REPORT. (THIS IS NOT A LATE FILING)
EMPLOYEE WAS WORKING AT THE UNDERGROUND CRUSHER WHEN A WASP STUNG HIM ON THE NECK.
ON 10-8-97 NIGHT SHIFT, A BOLTER OPERATOR REPORTED THAT BOLTS WERE BEING DRIVEN INTO THE RIB TOO EASILY IN 0-85-35. THE SUPERVISOR CHECKED THE AREA AND BARRICADED THE AREA TO PREVENT TRAVEL. T HE FOLLOWING EVENING, BETWEEN 7:00 PM AND 10:00 PM A GROUND FALL OCCURRED. THE FALL WAS REPORTED IMMEDIATELY TO MSHA INSPECTOR & VACAVILLE OFFICE WAS NOTIFIED. 10-30-97 DETERMINED REPORTABLE.

EE WA SUNLOADING 4" PIPE FROM A SUPPLY TRUCK. HE HAD ONE END OFF WHEN THE OTHER END SLIPPED DOWN. THIS CAUSED HIS END TO POP UP AND COME DOWN ON HIS LEFT HAND.
EMPLOYEE TOOK A DRINK OF POP FROM THE CAN. AS POP ENTERED HIS MOUTH, A BEE ALSO ENTERED AND STUNG HIM ON THE TONGUE.
WHILE REMOVING THE TAIL PULLEY GUARD FROM 1 CRUSHER BELT, THE GUARD TWISTED AND EE FELL AGAINST A CEMENT PILLER.
EE WAS OPERATING A FORKLIFT, UNLOADING A MOTOR CARGO TRUCK, WHEN HE DISCOVERED THE FORKS WERE TOO NARROW FOR THE TWELVE FOOT LONG CRATED FREIGHT. EE STOOD BESIDE THE FORKLIFT, UNKNOWINGLY PLAC ED HIS RIGHT HAND ON THE FORK EXTENSION RAIL. EXTENDED THE FORKS, AND BROKE AND LACERATED HIS SMALL FINGER TIP.
EMPLOYEE WAS WORKING ON A SOLENOID VALVE OF A HANKINSON AIR DRYER. THE DRYER WAS SHUT DOWN AND ISOLATED. WHEN HE UNSCREWED THE SOLENOID VALVE, BACK PRESSURE FROM A LEAKING ISOLATION VALVE BLEW THE PISTON OUT OF THE VALVE STRIKING HIM ON THE HAND, CHEEK AND NOSE.
OPERATOR OF HAUL TRUCK BECAME STUCK. AFTER ATTEMPTING TO FREE THE TRUCK, THE SUPERVISOR CAME BY.SUPERVISOR GAVE THE EMPLOYEE A RIDE ELSE WHERE FOR A CHOKER AND WHEN THEY CAME BACK THE GROUND F ALL HAD ALREADY OCCURED. THE GROUND HAD COME IN ON TOP OF THE TRUCK.
EMPLOYEE WAS REMOVING A BROKEN EYE FROM AN ENGINE CYLINDER FROM HT 3057 WHE HIS HAND SLIPPED STRIKING THE BROKEN EYE CAUSING A LACERATION TO HIS LEFT HAND.
EMP WAS DRIVING 992 LOADER WHEN A WASP FLEW INTOTHE OPERATORS AREA AND STUNG HIM ON UPPER LEFT ARM.
CLIOMB INTO THE PICKUP TO LEAVE AREA HE WAS WORKING. WHEN HE SAT BACK AGAINST THE SEAT HE WAS STUNG BY A YELLOW JACKET.
WORKER WAS TRAMMING A 6 YD LOADER FROM R-2 MAIN TO 175-4 BENCH, AS HE BEGAN TO ROUND THE CORNER INT 195 DRIFT, HE ENCOUNTERED THE POWDER TRUCK WHICH WAS PARKED IN A XCUT. WORKER'S VEHICLE STRU CK THE FRONT CORNER OF THE POWDER TRUCK SEVERING THE FUEL TANK. (THERE WAS NO DAMAGE TO THE FUELD TANK ITSELF).
WORKER WAS DRIVING A LOADED HAULPAK WHEN HE RAN THROUGH THE DYNAMICS CAUSING A SHEEL FIRE. WHEN HE CAME TO STOP, HE EXTINGUISHED THE FIRE, HOWEVER, BY THIS TIME THE FIRE HA DSPREAD TO THE SURR OUNDING GRASS AND BRUSH ALONG THE SIDE OF THE HAUL ROAD. THE FIRE BURNED FOR APPROX. 45 MINUTES.

EE WAS WITHDRAWING THE DRILL STEEL FROM A DRILL HOLE IN THE BACK. THE MACHINE WAS ROTATING DUE TO HOLE BEING RAVELY. EE HAD CUT AIR PRESSURE OFF TO THE LEG COMPLETELY WHEN THE STEEL HUNG. WHEN THE STEEL HUNG, IT CAUSED THE LEG TO RELOCATE AND AT THE SAME TIME, THE STEEL FREED ITSELF, ALLOWING THE MACHINE TO FALL AND STRIKE EE ON THE FOURTH TOE OF HIS RT FOOT CAUSING A FRACTURE TO T
