Showing all 11Miner was getting down from the loader cab at the end of EE's shift when EE's hand slipped. EE lost EE's footing on the ladder and fell against the loader fender.
When the two employees went to set the screen down onto the platform, one of the employees left hand got caught up on the left top corner of the screen slicing through the glove EE was wearing through EE's left middle finger and EE's left ring finger. The injury resulted in 9 stitches to the left hand.
Cleaning bucket on loader. Hydraulic was leaking causing it to lower slowly. Lowered onto foot causing injury.
While climbing down equipment ladder,L hand missed rung while moving feet to the next rung. Fell backwards approx 6 feet, landing on catwalk.

pulling 250# tail pulley back with come-along, to line up bearing. latch didn't catch & released itself, pulley moved forward pinning L hand between frame & pulley. Caught L middle, ring & little fingers.3 sutures on 3rd finger and 1 suture on 5th finger. Released for restricted, Right-hand duty 5/14/2015.
2 employees in 2006 Cat 735 seat belts fastened. Trainer was training employee on driving haul truck. The trainer w trainee driving 3rd load. Going down a steep road in 4th gear, truck jumped to 7th gear & speed increased w brakes on. Going too fast on curve, flipped onto its side. Both Employees have minor injuries. Investigating root cause of jumping gear.

While getting into cab of semi truck, driver "smacked" top of head on door frame. diagnosis - laceration and concussion without loss of consciousness.
He was in the pit, climbing off his bulldozer. There was a trench that he stepped on and he twisted his L ankle on the uneven surface. Ankle is swollen and doesn't move normally.
