Fall from machine

While climbing down equipment ladder,L hand missed rung while moving feet to the next rung. Fell backwards approx 6 feet, landing on catwalk.

Stepping off of dozer, twisted ankle. First aid only; then sought medical care on 09/29/2015 and was prescribed ankle brace and crutches.

Struck against a moving object

2 employees in 2006 Cat 735 seat belts fastened. Trainer was training employee on driving haul truck. The trainer w trainee driving 3rd load. Going down a steep road in 4th gear, truck jumped to 7th gear & speed increased w brakes on. Going too fast on curve, flipped onto its side. Both Employees have minor injuries. Investigating root cause of jumping gear.

While getting into cab of semi truck, driver ""smacked"" top of head on door frame. diagnosis - laceration and concussion without loss of consciousness.

He was in the pit, climbing off his bulldozer. There was a trench that he stepped on and he twisted his L ankle on the uneven surface. Ankle is swollen and doesn't move normally.
