Checking if chimney tube for building sand was plugged using a mirror. Radioed operator to open clam gate. Gate closed striking chin and left arm. Small cut on chin required stitches.
THE CREW WAS CHANGING CONE LINERS. WHILE INSTALLING THE NEW MANTLE LINER, EE FELL FROM A POORLY-DESIGNED PLATFORM AND LANDED ON A PORTION OF THE CRUSHER FRAME. AT FIRST IT SEEMED LIKE JUST A BRUISE. ON 3/7/05 EE SAW DOCTOR WHO SAID EE PROBABLY HAD TORN CARTILAGE. ON 3/14/05 WE LEARNED THAT X-RAYS CONFIRMED 2 FRACTURED RIBS.
Dump truck driver was assigned to deliver a solo load (truck only) & needed to leave his trailer at the Mine site. When jacking up his trailer, the driver needed to use excessive force on the crank handle to raise the trailer. With both hands on the crank handle, left hand over right hand, his right ring finger was fractured on the middle segment.
EE WAS MOUNTING HIS 992 LOADER WHEN HIS RIGHT ANKLE TWISTED DUE TO A LOOSE STONE ON THE GROUND.
EMPLOYEE WAS CLIMBING THE STAIRS TO THE CEMENT SILOS WHEN HE SLIPPED ON A STEP AND TWISTED HIS LOWER BACK.
EE WAS HEATING UP COUPLING WITH A ROSE BUD TORCH SO THAT THE COUPLING COULD BE REMOVED. 2 OTHER EES WERE STRIKING THE COUPLING WITH 20# DEAD BLOW MALLETS. ONE OF THE MALLETS GLANCED OFF OF THE COUPLING STRIKING EE ON THE RIGHT SIDE OF HIS FACE.
EE WAS WORKING ON NEW TYPE POSITIONING VALVE ON #44 SILO. THE VALVE HAD TO REMAIN ENERGIZED DUE TO THE WORK THAT WAS BEING DONE. EE BEGAN THE CYLINDER TO THE VALVE HE FORGOT TO SHUT OFF THE AIR THAT POWERED IT. WHEN HE STUCK HIS FINGERTIPTHROUGH THE CLEVIS TO ALIGN EVERYTHING THE CYLINDER RETRACTED CUTTING OFF FINGERTIP. NO BONE INVOLVED.
EE REACHED BACK TO PICK UP A GUARD AND BUMPED HIS RIGHT ELBOW ON A HANDRAIL SHORTLY AFTERWARDS HIS RIGHT ARM BEGAN TO SWELL.
EE WAS GETTING OUT OF HIS TRUCK WHEN HE TWISTED HIS LEFT KNEE.
OUTSIDE/SWINGING A SLEDGE HAMMER TO DRIVE A PULLEY SHAFT BACK INTO BEARING HOUSING #3 FEED WEIGHT DISPLACED ON LEFT LEG/HEARD AND FELT KNEE SNAP/IMMEDIATE PAIN TIGHT ENCLOSED AREA.
EMPLOYEE WAS CLOSING OVERHEAD GARAGE DOOR AND WHILE PULLING ON THE CHAIN RIGHT LITTLE FINGER GOT CAUGHT IN THE CHAIN AND TWISTED. INJURY BECAME REPORTABLE ON 9/4/01. DOCTOR PUT A CAST ON HIS R IGHT HAND.
EE WAS CUTTING A PIECE OF RUBBER BELTING WITH A RAZOR KNIFE. WHILE PULLING THE KNIFE TOWARD HIM IT SLIPPED AND HE STABBED HIMSELF IN THE LEFT FOREARM.
EMPLOYEE WAS STANDING 2" BY 12" BY 5 FT OAK BOARD JACKHAMMERING CLINKER INSIDE A RR CAR. HE HANDED THE JACKHAMMER TO ANOTHER EMPLOYEE WHEN THE BOARD SPLIT CAUSING HIM TO FALL. HE REACHED OUT T O BRACE HIS FALL SEVERELY SPRAINING HIS LEFT ELBOW.
EMPLOYEE HAS BEEN OPERATING A FORKLIFT OUTSIDE WHERE IT HAD BEEN RAINING. HE ENTERED THE PLANT STARTED DOWN THE RAMP. FORKLIFT SLID INTO PALLETS OF BAGGED MATERIAL. HE STUCK OUT HIS LEFT ARM TO BRACE HIMSELF. HE BROKE HIS LEFT ARM ON IMPACT.
WHILE REPAIRING OIL LEAK ON NORTHWEST HYDRAULIC CYLIDER WHICH WAS UNDER PRESSURE IT STRUCK HIS FOREARM AND ELBOW CAUSING A BRUISE.
EMPLOYEE AND TWO OTHER EMPLOYEE'S WERE INSTALLING A WERA SLEEVE ON SHAFT WITH A HAMMER AND A PIECE OF BRASS. WHILE THE OTHER EE HIT THE BRASS PIECE ON ONE SIDE, IT SLIPPED AND HIT EE'S RIGHT T HUMB.
EE WAS MAKING A WIDTH AND HEIGHT ADJUSTMENT TO THE SOUTH BAG GUIDE AND WHILE BENDING OVER THE EQUIPMENT FELT A POP.
WHILE SLAG DROPPED AND LODGED ON THE OUTER TONGUE OF BOOT WHICH BURNT A HOLE CAUSING PENETRATION OF THE SLAG INSIDE. THIS CAUSED A COMBUSTION REACTION INSIDE THE BOOTS WHICH CHARRED SOCK AND B OOT THEN CAUSED SKIN DAMAGE.
WHILE OPERATING AN END GRINDER, WITH A CUT OFF WHEEL ATTACHMENT, THE GRINDER KICKED BACK AND THE WHEEL LACERATED THE ARM.
EMPLOYEE STEPPED ON A TOOLBOX HE LEFT ON WALKWAY ON THE DRAGLINE AND SPRAINED HIS LEFT ANKLE.
EE WAS PICKING UP AN OXYGEN BOTTLE AND FELT A STRIAN IN THE GROIN AREA. CONTINUED TO WORK UNTI 7/13 WHEN HE REINJURED IT BY PICKING UP A BEAM.
EE WAS CARRYING A 96 LB BAG OF CEMENT UP A WALKWAY AND SLIPPED, CAUSING A SHARP PAIN IN THE GROIN AREA.
EE WAS TYING REBAR WHEN THE PLIERS SLIPPED OFF THE WIRE AND STRUCK HIM IN THE LOWER LIP.
EE WAS LIFTING MATERIALS AND FELT A PULL IN THE GROIN AREA.
WHILE ADJUSTING A JACK STAND, THE JACK SLIPPED AND PINCHED EMPLOYEE'S 2 FINGERS BETWEEN THE PIPE AND THE STAND.
EE STEPPED ON OPERATIONAL WELDING CABLE WHICH HAD EXPOSED WIRES. THE CABLE WENT TO GROUND WHICH POTENTIALLY SENT A SHOCK TO THE EE AS HE STEPPED ON IT CAUSING HIM TO FALL BACKWARDS AND ONTO HI S SIDE.
EE CLIMBED ON TOP OF SCAFFOLD TO USE AS AN ACCESS INTO MILL. WHILE CLIMBING THROUGH THE HOLE, HE FELL TO THE CONCRETE FLOOR, LANDING ON THE WRIST/BACK.
EE STEPPED ON A HOLE COVER AND FELL THROUGH IT. EE LANDED ON CONCRETE FLOOR AND THE COVER LANDED ON TOP OF HIM.
EMPLOYEE WAS WORKING IN A SCISSOR LIFT IN A MANBASKET. HE LEANED OVER THE EDGE OF THE BASKET ANDACCIDENTLY HIT THE ELEVATION SWITCH. IT CAUSED HIM TO BE ELEVATED AND PINNED HIM BETWEEN THE LIF T BASKET AND A PIECE OF ANGLE IRON BRACED ABOVE HIM.
EE WAS PULLING ON SOME WIRE WHEN HE FELT SOME WIRE WHEN HE FELT SOME PAIN IN THE GROIN AREA.
EE WAS WORKING AT AN ELEVATION, THREADING CONDUIT. HE WAS PULLING DOWN ON THREADER HANDLE. THE HANDLE CAUSED HIM TO FALL BACKWARD AND HIT THE LEG ON SCAFFOLDING.
EE WAS INSTALLING TRASH SHOOTS, LIFTING SHEETS TO ALIGN THEN TO BE WELDED. EE FELT PAIN IN THE LEFT GROIN AREA.
EE WAS HGRINDING METAL. HE LIFTED THE SHIELD AND GLASSES AND SOME MATERIAL FELL INTO THE EYE.
EE WAS LIFTING A METAL DISCONNECT STAND THAT WEIGHED APPROX. 100 LBS. AND FELT SOME DISCOMFORT IN THE GROIN AREA.
