Employee was working on the F780 clinker/slag conveying system. Employee looked up into the discharge chute of the F780 dust collection system to evaluate the condition of the chute and observe the condition of the dust collector bags. Employee knocked loose cement/slag dust which fell into employee's left eye. The employee was wearing safety glasses.

Employee was working in the No. 1 Finish Mill on a ladder lifting a liner above employee's head. The legs of the ladder were on top of mill balls and as the employee lifted the liner the ladder sank into the mill balls. The employee reacted by pushing harder on the liner and injured right shoulder.

While installing the lower portion of a chain guard, the employee placed fingers between the guard and floor grating. The guard slipped pinching employee's left ring finger causing a laceration requiring 16 stitches.

Two maintenance mechanics were disassembling a bucket elevator. One mechanic was holding a punch in place with a tool holder while the other mechanic struck it with a sledgehammer. A piece of metal that had mushroomed around the end of the punch sheared off and cut the left thumb of the mechanic holding the tool holder. The EE received three stitches to close the wound.

EE was installing an air knife on the alt fuels belt when the air knife shifted approximately 4" and struck EE's left eyebrow causing a 1" laceration. The air knife structure has no sharp edges and weighs approximately 3 pounds. The employee received 6 stitches and returned to work immediately with no restrictions.

EE jumped down from a storage trailer that was approximately 4 feet in height. When EE landed, left knee gave out. After visiting the doctor following the incident, EE was returned to work with restrictions that still allowed EE to perform EE's job duties. On August 11th, EE had an MRI conducted and it was discovered that EE had a torn ACL and meniscus in left knee.

Employee was injured while installing a sprocket on a drive shaft in the maintenance shop. EE was striking the sprocket with a hammer when a piece of metal approximately 1/4" in size sheared from the sprocket and stuck into EE's right arm. EE received one stitch to close the wound. EE was wearing gloves, safety glasses and a hard hat.

EE was injured while opening the hatch doors on the truck sander. EE tried to stop the hatch door from falling and hitting another employee and in doing so pinched the pinky finger on right hand between the hatch door and the door jam. EE was wearing work gloves.

While vacuum cleaning interior of the refractory line duct an employee's hand became lodged in vacuum hose. The truck was shut down and the employee evaluated. Corrective Action: Vacuum breaker and hose handle to be installed on all vacuum operations involving personnel handling the flex hose.

EE was connecting a trailer to the Maintenance pickup truck. EE pulled the tongue of the trailer towards the ball-hitch and pinched EE's finger between the trailer and the ball-hitch, lacerating the ringer finger on EE's left hand. EE returned to work shortly after receiving 7 stitches.

Employee was carrying chain fall in their arms and over their shoulder. Due to unsafe ice conditions on the walking surface of the normal route, employee had to step over pipe and caught their foot in the chain, falling onto their arm and resulting in a compound fracture of the left radius.

Employee positioned a steel frame on a sawhorse so they could paint it. Frame became unstable and tipped over. Employee grabbed it and the edge of the frame lacerated the left forearm. Laceration required 16 stitches to close wound. Employee came back to work after being stitched.

Employee went directly to coal screw area at start of shift to ensure no blockages. Within a couple minutes of arriving at coal screw, employee radioed that ee needed help. Supervisor found ee laying on the grating stating that ee had injured hand. Employee had lost the tip of one finger and lacerated two others. Employee stated ee didn't know how it happened.

Tightening a nut and bolt with a spud wrench and the wrench slipped and hit EE in the nose. Doctors are saying that it is suspected to be broken. The practitioner report says that this person is on 'modified duty'. Modified to include limited lifting, minimize bending and no heavy pulling or pushing. This person has been off of work since the injury happened.

Employee had just completed a cleaning task and sat down to wipe sweat from his face. He had taken his safety glasses off and a gust of wind blew some dust in his eyes. We sent him to the emergency room to get his eyes flushed and the Dr. requested that the employee remain out of work until Monday morning. scheduled shift on 11/11/15.

Our employee was handling a beam on our job site at the plant. He got his hand caught briefly between pinch point and as a result received bruising and a cut on the back of his hand that required stitches. He went to the nearest emergency facility, was treated and returned to work.

Employee was using a come-along to move over a screw auger trough with a wire rope sling wrapped around it. Whilst pulling the lever the wire sling broke and the employee fell backwards and hit his upper back on the edge of a steel beam. This contact caused a contusion on his back. The doctor's note stated "ee was seen today in the ER. He should not work tomorrow".

Just as the employee cutting the board was finishing the cut, the injured employee got his hand too close to the blade of the saw and the blade nicked his finger on his right hand. EE was working with another EE to remove the headwheel assembly of the 781 drag conveyor using 2 chainfalls. While the load was partially lifted, EE reached around the headwheel assembly to retrieve a wedge and the headwheel assembly shifted. When it shifted, it pinned his arm against a beam causing bruising to his right arm.

EE was using a jackhammer to clean inside a hopper when he decided to sit and take a break to wipe sweat from his head. He removed his hard hat and gloves and wiped sweat off. When he stood up, he slipped. When he slipped, he grabbed onto a piece of metal in the hopper to prevent the fall and the metal cut his left hand. The laceration required 5 stitches to close.
