Employee had a seizure was not injured. We did call a ambulance and ambulance came and took EE to the ER in Abilene, Tx. Waiting to hear back on if it was due to prior health issues.

Showing 50 of 102Crane Employee was unloading slab from cutting floor and was putting strap on stone slab when a slab that was around 6" away had broke fell on the left ring finger of employee and broke tip of finger. The employee had hand by another stone slab putting strap on another slab when the one that broke fell over striking employee's finger.
Operator was stepping off of tractor at the end of the shift. And operator stepped to far back and heal partially caught on roughback strip causing employee to twist left ankle.
Employee was flipping a stone on roller table to push thru to chopper. When employee flipped stone he hit his right pinkie finger between stone and roller. Broke tip of pinkie and took chunk of pinkie out from under the finger nail.
Employee is a chopper Op. on this morning he was chopping stone. on the event. He had his hand on stone and went to bring head down to chop and the frame of the head of the chopper come down on his left hand and broke his left index finger and middle finger.
EE was unloading heal to put into trash bucket. The heal usually is 1"-3" thick. The EE was trying to break heal when 2-3.5" strips fell over on his right hand and thumb. Dr's note soft tissue bruise.
Dry cracks in the rock, was palleting stone and one had a dry crack and broke landing on his thumb and breaking thumb. EE was wearing gloves.
Rock fell over on top of his right foot.
Armando was trying to flip a hill. The hill fell backward and fell on his middle right finger. Finger was caught between hill and side rail.
Slipped in drain ditch, hit his left shoulder on a large rock.
Moving a rock to its side and pinched his left testical.
Employee was trying to move a rock on the flipper table. Started to fall and put his and on flipper table and twisted his left wrist.
Went to jump off his skid loader and his knee popped.
Employee was on Kawasaki loader when another employee found him slumped over the steering wheel. He wasn't responsive and CPR was started. Ambulance was called. EMS continued CPR and employee was pronounced dead at the hospital from a heart attack.
Employee was hammering rocks for bulk pile. He finished one rock, threw it in the bucket. As he reached for another rock, another employee threw another rock on the table. His pinkie finger was struck and sliced open at the first segment. He was taken to the ER and stitches were given. Employee was placed on light duty until doctor release.
Employee was pushing rock through chopper window when he smashed his finger between two rocks. After reporting to the office for first aid, he was sent to the Dr for stitches. Employee received 6 stitches and was released to return to work. He returned to the doctor in one week to remove stitches.
Employee was manually pushing rock on top of the saw table. His right ring finger was pinched between the wooden table and the rock. The tip of his finger was split open. First aid was given and determined a doctor should examine his finger. Upon doctor evaluation employee received 7 stitches.
Employee was moving rock from roller table and stacking it on pallets. his left hand was smashed between two rocks when a co worker prematurely pushed the next rock down the line. His left ring finger was broken. He had surgery 1/22/08 to insert a pin in his finger.
EE was moving rock when he slipped on a water puddle and twisted his left ankle. He was sent to the doctor and instructed to stay off of his ankle until the swelling decreases.
Employee was carrying rock to the trash bin. He jumped on a metal step and fell through the step. He twisted his right knee as he fell. There was slight swellig and immediate pain. Upon dr. examination his knee was found to be strained. He was directed to stay off of it for a day then return to work.
Employee was moving rock into a bucket when he heard and felt his back pop. There was immediate pain. When walking he feels a painful twitch. Employee was sent to the doctor for an exam.
Employee was lifting rocks and moving them to the opposite side of his saw. He felt a pull or strain across his chest. He awoke the following morning and could barely move. He reported the injury on 8/16/07. Upon his visit to the doctor, employee was diagnosed w/muscle pain and referred to his family doctor for follow up.
EE was stacking pallet, when another EE tossed a rock and smashed his finger. Left middle finger had a laceration on the finger tip. Treated with first aid and sent to doctor. Steristips applied and told to stay off work for one week.
Employee was working the window at the chopper. He was pushing rock through the window & placed his left hand on a roller. The roller was spinning and his left hand was pulled between rollers. Pulled his hand out, but middle finger was injured. Referred to specialist who cut bone back to 2nd knuckle of middle finger.
EE was helping to load rock onto table. The crane moved, causing the strap to catch his left hand. EE had a deep laceration to the pad of his palm. Immediate first aid was provided and taken to the ER for treatment. He received 12 stitches. On light duty until he sees a specialist.
EE was stacking a pallet, when a rock fell off and struck his right knee. There was immediate pain and slight swelling. He came to work the next evening and then worked his entire shift. His knee was extremely swollen and painful. He then called to report his injury on 6/28/07, was sent to clinic for knee strain and pain.
EE was helping to hand load a bucket of rock on the load. He stepped onto the bulk pile and lost his footing. EE felt immediate pain in lower back as he fell into the rocks. He took ibuprofen @ 10:20 to relieve pain and possible swelling. Doctor's appointment was made for eval.
Employee was stacking rocks when his hand got stuck between two rocks. After removing the top rock and releasing his hand, the tip of his right middle finger appeared to be broken, bent and unable to flex. Upon doctor's examination, it was found to be broken.
Employee was moving a rock w/crane. The rock shifted in the strap and the ee lost control due to momentum. His right leg was pinned between the rock and the wall of the building immediate swelling and bruisng upon dr. visit, he was diagnosed w/torn tendons in his right shin/calf.

EE was cleaning up to go home when he started to feel pain and discomfort in R shoulder and upper middle back. He reported his possible injury to the office at 5:45 pm 8-9-06. Upon doctors visit he was advised to rest and return to work the following Monday. It was apulled muscle.
Employee was reaching beside his seat while on his skid steer when he cut his left middle finger knuckle on a piece of metal under the seat. Upon doctor's visit, he received 2 stitches.
EE was stacking rock when another employee smashed his left thumb between another rock and the one he was placing.
Employee was moving a rock along the roller of the chopper when he caught his left pinkie between the rock and the roller. He cut his finger on the rock. Was taken to the ER and received 5 stitches.
Employee was turning a rock when he twisted his right hand. He felt an immediate pain. Upon the doctors visit, a fractured bone was found in the palm of his hand.

Employee was pushing rock through chopper when he felt a pain on his left side under his rib cage. He was sent to the doctor where he was referred to another physician for followup for possible hernia or pulled muscle.
Employee was running a tractor at the quarry when another employee noticed his tractor lost control and was going to hit a structure. Upon inspection the employee was found to be unconscious. First aid and CPR was begun. Ambulance was called. Upon EMT arrival and work to revive, employee was pronounced dead.
Another employee was picking up a rock from the floor with skat track. The rock rolled off the forks and on to ee's left foot. Immediate swelling and pain. Was taken to ER given a shot and told to return for xray when swelling reduced.
ee was throwing rock into trash bin, when the rock broke & hit him in the middle of the back. Ee did not report the injuryuntil 4/25/06 at 1:30pm.
EE had been operating a skat track then got off to help stack a pallet. As he bent over to pick up a rock, he felt a pain in his lower middle back.

Employee reported his injury on 11/4/05. His back had been hurting him for a while. He had not been wearing a back belt at work. After a few days he started to hear his back belt. The back pain persisted and he decided to report the injury and request a doctor;s appointment.
EMPLOYEE WAS PUTTING ROCK ON THE TABLE. HE SMASHED HIS LEFT PINKIE BETWEEN THE ROCK HE WAS CARRYING AND THE ROCK ALREADY ON THE TABLE. PINKIE WAS SWOLLEN AND BRUISED. THE FINGERNAIL WAS POPPED OUT.
Employee was working at the chopper table when a rock fell off the table. The rock hit his foot right behind the steel toe of the left foot.
EMPLOYEE WAS PUSHING BLOCK THROUGH THE CHOPPER WINDOW WHEN HE FELT A PULL IN HIS BACK. HE WAS WEARING A BACK SAFETY BELT.
Employee was cleaning chopper. He was holding up the table to clean underneath and the table slipped smashing his left hand. His middle finger and ring finger tips were severed. Upon hospitalization, the middle finger was re-attached.
Employee was working outside when he became extremely overheated. Employee poured an ice cold cup of water over his head. Within moments the employee fainted. He was transported to the office. He had trouble maintaining lucidity.
