Uneven ground at base of ladder, employee stepped into a pothole.
Employee was operating plant. A rock became lodged in a conveyor belt. Employee used a pry bar and hammer to dislodge rock. Hammer struck pry bar, pry bar struck employee's finger.
EMPLOYEE WAS SHOVELING SPILLAGE FROM AROUND PLANT AREA, SOME TIME LATER HE COMPLAINED OF IRRITATION IN HIS ELBOW.
EE WAS WORKING ON 39L DOZER, REMOVING TRACK FROM DOZER, HE RESTED HIS LEFT HAND ON TRACK WHEN UNBOLTING, TRACK SEPARATED AND PINCHED RING FINGER BETWEEN PADS ON TRACK. HE LOST HIS NAIL AND SKI N OFF TIP AND SIDE OF FINGER.
