Employee was in the work boat and working on the back of the dredge. The employee tried stepping on to the back of the dredge hitting EE's head on the exhaust pipe attached to the dredge motor. This results in the employee falling back into the boat striking EE's rear end on two guide bolts.
349F excavator was trying to dig a big rock out of pile against west wall, operator was trying to find the top edge of rock to pull rock from pile. The teeth slipped off the rock which caused machine to jar hard against the pile. The operator didn't notice any discomfort until operator got out of machine at end of day and stood up.

Installing drive belt around sheave and cut finger on edge of sheave

Aluminum ferel coupling pushed off end of 4' lay flat hose,which struck employee in right ankle.
Independent non-employee tri-axle driver reported to scale house with 3 severed finger tips on right hand. Driver pulled to dump off area, proceeded to lighten loaded material. Driver was cleaning off tailgate, middle, ring and pinky finger tips were pinched between body and tailgate of truck, resulting in removal of finger tips. Driver was transported to local ER by lab tech.
employee notified supervisor fuel filter on filling station was clogged. employee was told not to perform task of changing fuel filter. employee performed task by using half of an extension ladder to climb to shut off valve on fuel tank. employee climbed ladder,which slipped out from under their feet. which lead to a fall approx 6 feet, bruising right ankle.
Employee was removing a drive shaft from a pickup truck, when EE's hand slipped off and got caught in the universal joint and put a deep cut on EE's pinky finger.
