Aluminum ferel coupling pushed off end of 4' lay flat hose,which struck employee in right ankle.

Caught in, under or between a moving and a stationary object

Independent non-employee tri-axle driver reported to scale house with 3 severed finger tips on right hand. Driver pulled to dump off area, proceeded to lighten loaded material. Driver was cleaning off tailgate, middle, ring and pinky finger tips were pinched between body and tailgate of truck, resulting in removal of finger tips. Driver was transported to local ER by lab tech.

Fall from ladders

employee notified supervisor fuel filter on filling station was clogged. employee was told not to perform task of changing fuel filter. employee performed task by using half of an extension ladder to climb to shut off valve on fuel tank. employee climbed ladder,which slipped out from under their feet. which lead to a fall approx 6 feet, bruising right ankle.

The incident happened to a Non Quarry Employee. ee was an outside truck driver that slipped off of truck and injured leg while in the quarry at the scale location.

Struck by flying object

IEE(injured) stopped water truck to ask another EE (mechanic) a question and at that time EE's were changing teeth on the Hitachi 330 track hoe. A jammed tooth required a hammer to loosen it. Mechanic hit the tooth with hammer when a piece of hammer broke off the edge and hit EE in the left bicep (which EE was standing 10' away)

Struck by falling object

While working the track mounted portable crusher/screen plant back into operating position with the remote control box, a rock fell from the portable plant striking the insured on his head cutting his scalp. He said his hard hat had fallen off previous to him moving the plant and that he did not put it back on.

Operator was attempting to push the Breaker Switch into the ""ON"" position after a bad electrical storm, when the breaker blew up and burnt his right hand.

Employee was attempting to get into the wheel loader with muddy boots. When he got onto the step or ladder of the loader, he slipped and hit his right knee on the step and fell to the ground. He did not seek medical attention until 11/20/07. He was treated & released.

EE was using a hammer to repair a roller pin and was putting the bearing back onto the pin. When he went to strike the pin with the hammer he missed the pin and hit his left left hand that was holding the pin.

EE WAS SHOVELING MATERIAL UNDER THE #3 SCREEN DECK WHEN A ONE FOOT DIAMETER ROCK FELL OFF AND STRUCK THE EE ON HIS LEFT SHOULDER AND BACK.

WHILE LIFTING A 150 LB STEEL IMPACTOR HAMMER TO REPLACE BACK INTO THE STONE CRUSHER EE PULLED HIS RIGHT KNEE CAP. IT IS UNDETERMINED IF THIS IS A REPORTABLE INCIDENT DUE TO THE FACT HE LOST NO DUTY TIME AND DID NOT TAKE MEDICATION. HE DID NOT GO FOR MEDICAL TREATMENT UNTIL FRIDAY AFTER WORKING TWO FULL SHIFTS.

Over-exertion in pulling or pushing objects

EMPLOYEE WAS PULLING A HOSE WHEN THE HOSE GOT STUCK ON A ROCK AND HE PULLED AND STRAINED HIS SHOULDER. THE EE CONTINUED TO WORK AND DID NOT REPORT THE INJURY OR SEEK MEDICAL ATTENTION UNTIL 7-22-02. IT IS UNDETERMINED IF THIS INCIDENT IS A REPORTABLE INCIDENT DUE TO THE FACT THE EE DID NOT LOSE ANY TIME OFF, AND HE TOOK NO MEDICATION.

Struck by falling object

WHILE CLEANING OFF THE SCREEN DECK THE LABORER PLACED HIS LEFT HAND ON THE CATWALK STEP TO BRACE HIMSELF, WHEN A ROCK ROLLED OFF THE BELT AND STRUCK HIS LEFT HAND. EE WAS TERMINATED DUE TO FACT HE NEVER CALLED INTO WORK OR CAME BACK TO LIGHT DUTY WORK.

EE WAS GRINDING A JAW DYE WITH A GRINDER AND THE GRINDER SLIPPED AND CUT HIS RIGHT KNEE. THERE WAS NO DAMAGE TO ANY EQUIPMENT, ONLY A KNEE INJURY.
