Employee was using an angle grinder to cut a loose bolt on a board. The board was positioned on the back of a flatbed truck. Due to the bolt being loose, when the disc made contact it caused the grinder to kick into employee's hand. This resulted in a laceration requiring 3 stitches. Employee was wearing gloves.

Shop mechanic was removing a u-joint from a 980 loader when the wrench slipped and hit EE in the mouth. The bottom half of EE's front tooth was broken off. EE received orthodontia work recently making it a reportable incident.

Electrician tried to manually manipulate a locked out electrical motor via the "V" belts and got fingers pinched in the flywheel.

EE was hosing down the wash plant pad when EE felt a pop in shoulder.

Material slough, partial engulfment, follow up appointment revealed hernia.

A ROOFING CONTRACTOR was trimming some material with a razor knife and the knife slipped, cutting the CONTRACTOR on hand.

Accident was reported on the above date, we were not aware of it becoming reportable until 9/6/16. Doctors notes were not provided until 9/6/16, as employee was on a pre-scheduled vacation the week prior.

Miner finished adjusting the tension on the feeder belts from a ladder. He was descending the ladder with his back to the ladder when a button on his coveralls hung up on the motor mount. He stepped up a rung to free the button and his foot slipped and he fell approximately 3 ft. to the ground below.

Haul truck driver suffered a back and neck strain during operation of a haul truck and at the time of being loaded with rockery rock from a front shovel. After loading the truck with 2 large rocks, the shovel operator started to swing and a 3rd unexpected rock rolled out of the bucket and struck the bed of the haul truck jolting the driver.

While walking to the shop to attend the morning tool box talk a miner suddenly collapsed due to an unknown personal medical condition and hit his head on the concrete floor. The miner sustained a laceration which required 6 stitches to close.

Worker strained back while pulling a 5 gallon bucket of water up onto an elevated walkway to prime a pump on April 17. He continued to work his normal schedule until May 2nd. On May 3, he went into the doctor due to ongoing back pain and was prescribed a prescription muscle relaxer, physical therapy and released to full duty. He returned to work on his next scheduled shift.

Employee was turning the cone bowl on and caught his right knee between the bowl and the adjustment arm.

Employee had been swinging a 10 lb hammer on and off for a few hours when he experience a burning sensation in his right elbow. The pain went away after his shift but when he started his next shift the pain came back.

Employee was working in a feeder tunnel trying to straighten a plastic tube. He didn't feel any pain initially but started to hurt a few hours later.

Employee's right elbow is starting to bother him after years of repetitive motion of operating a drill.

Employee was operating a disc grinder under a cone belt. The grinder became jammed and kicked back, striking the employee's left wrist creating a laceration.

Employee was loading and off-loading blasting supplies (approx 50 lb bags) from storage machine to a pick up truck and from the truck to drill holes at the quarry bench. Employee could not identify a particular incident or event of injury but started to feel pain in his hip that continuously got worse during the day.

Hearing impairment from normal job related activities.

Employee was climbing up on a ladder when he lost balance and fell backwards 3 to 4 feet and landed on wrist and hit head on truck.

Employee was carrying a 20 ft extension ladder when a wind gust caught ladder causing employee's back to twist.

Employee was grinding in a manlift. Protective eye wear slipped down and he got a small splinter in the eye.

Employee had sand in eye resulting in scratched cornea. Employee unsure when sand got in eye, it may have been during clean up around the wash plant or having rubbed his eye with a dirty glove.

Employee was getting ready to dismount truck and slipped on the catwalk and cut his leg requiring stitches.

Employee was welding on the jaw crusher when a piece of metal got into his left eye. Employee was wearing safety glasses and face shield. Continuing irritation required doctors visit to remove object and treatment with an antibiotic prescription.

Employee was cutting a 1/4" plate with a torch when a piece of slag entered his right nostril. Employee notified supervisor 9 days after the incident as his nostril remained swollen. Medical treatment was sought and an anitibiotic prescription was given.

Employee was attempting to loosen a drain plug underneath a front end loader. As he was pulling on the wrench, it suddenly loosened, and his momentum caused him to hit the back of his head on a tire rim. Employee was knocked unconscious and taken to the hospital for observation. Tests were negative for any head injury.

Employee was swinging a 12 pound hammer to remove the main shaft from a water pump when he recieved a partial tear of his right biceps tendon.

While using a hole punch to punch holes in a piece of rubber, employee hit finger with hammer, requiring stiches.

Employee strained tendon in his left leg as he was working on changing a conveyor belt.

Worker was lifting steel door into the syntron of the pit run tunnel when he strained his back. Worker is receiving therapy and on prescription medication.

During a trip to Oregon for work-related training, worker stepped awkwardly on the side of a curb. Doctor diagnosed it as a foot sprain and prescribed pain medication.

Tripped over metal rails that are inbedded in shop floor. Fell to ground, bruising hands.

EMPLOYEE WAS MOUNTING A TOOLBOX ONTO A SERVICE TRUCK. WHEN HE PUSHED THE TOOLBOX, THE LID CAME DOWN ONTO HIS LEFT INDEX FINGER, LACERATING AND BREAKING IT.

STEPPING DOWN FROM A CONCRETE SLAB TO THE GROUND. HE TWISTED HIS ANKLE, THERE WAS ABOUT AN 8" DROP FROM THE SLAB TO THE GROUND.

WHILE MOVING SOME PIPE HE SMASHED HIS SMALL FINGER.

HEARING LOSS IN BOTH EARS.

WHILE REPLACING A TROUGHING ROLLER HE STRAINED HIS BACK

WHILE CHECKING UNDER THE HOPPER FEED BELT, HE HIT HIS LEFT SHIN ON A SHAPE CORNER OF A GUARD.

WHILE LIFTING A PIECE OF DRILL STEEL FROM THE GROUND TO PUT RT BACK ONTO THE DRILL HE STRAINED H LOW BACK.

WHILE CHANGING OUT SCREENS EE LIFTED THE ONE END OF THE SCREEN AND THEN TURNED THE SCREEN SO HE COULD HOOK THE END OF THE SCREEN TO THE BOOM TRUCK HOOK TO RAISE IT OUT AND IN. HE TWISTED HIS R IGHT KNEE. NOTE: HE DID NOT SEE A PHYSICIAN FOR THIS UNTIL 7/26/01.

LOSS OF HEARING DUE TO WORKING HABITS.

A PICKING EYE WAS WELDED TO MANGANESE AND THE WELD BROKE. THE BOOM TRUCK BLOCK THEN HIT EE ON THE LEFT SIDE OF HIS FACE.

OVER A PERIOD OF TIME THE EE HAS SIGNS OF DEVELOPING CARPAL TUNNEL IN HIS WRIST

WHILE WELDING A PIECE OF ANGLE IRON HIS LEFT ARM SHIRT SLEEVE CAUGHT ON FIRE AND BURNED HIS ARM. THE BURN IS PROBABLY AND INCH IN DIAMETER.

EE HAD HIS RIGHT HAND RESTING ON A SOLID PIECE OF MACHINERY WHEN THE VBELT SWING AWAY GUARD SWUNG BACK AGAINST HIS HAND

EE WAS REMOVING METAL BANDING ON HAND RAIL OF TUNNEL CONVEYOR WITH A CRESENT WRENCH. THE WRENCH SLIPPED AND THE BANDING CUT HIS LEFT HAND INDEX FINGER REQUIRING 4 STITCHES AND A TETNUS SHOT. A PROPER CUTTING TOOL AND GLOVES WOULD HAVE PREVENTED THE INJURY.

THE EE JUMPED OFF OF A FLAT BED WORK TRUCK WHEN HIS FEET HAD THE GROUND IT SPRAINED HIS LEFT KNEE.

EE WAS OPERATING AN EXCAVATOR AND ATTEMPTING TO BREAK A HUGH BOULDER. HE HAD THE FRONT OF THE TRACKS ELEVATED OFF THE GROUND AND THEN WHEN THE BOULDER BROKE THE MACHINE CAME DOWN HARD TO THE G ROUND. THE SEAT OF.
