Employee was using an angle grinder to cut a loose bolt on a board. The board was positioned on the back of a flatbed truck. Due to the bolt being loose, when the disc made contact it caused the grinder to kick into employee's hand. This resulted in a laceration requiring 3 stitches. Employee was wearing gloves.

Shop mechanic was removing a u-joint from a 980 loader when the wrench slipped and hit EE in the mouth. The bottom half of EE's front tooth was broken off. EE received orthodontia work recently making it a reportable incident.

Miner finished adjusting the tension on the feeder belts from a ladder. He was descending the ladder with his back to the ladder when a button on his coveralls hung up on the motor mount. He stepped up a rung to free the button and his foot slipped and he fell approximately 3 ft. to the ground below.

Haul truck driver suffered a back and neck strain during operation of a haul truck and at the time of being loaded with rockery rock from a front shovel. After loading the truck with 2 large rocks, the shovel operator started to swing and a 3rd unexpected rock rolled out of the bucket and struck the bed of the haul truck jolting the driver.

While walking to the shop to attend the morning tool box talk a miner suddenly collapsed due to an unknown personal medical condition and hit his head on the concrete floor. The miner sustained a laceration which required 6 stitches to close.

Worker strained back while pulling a 5 gallon bucket of water up onto an elevated walkway to prime a pump on April 17. He continued to work his normal schedule until May 2nd. On May 3, he went into the doctor due to ongoing back pain and was prescribed a prescription muscle relaxer, physical therapy and released to full duty. He returned to work on his next scheduled shift.

Employee was cutting a 1/4" plate with a torch when a piece of slag entered his right nostril. Employee notified supervisor 9 days after the incident as his nostril remained swollen. Medical treatment was sought and an anitibiotic prescription was given.

Employee was attempting to loosen a drain plug underneath a front end loader. As he was pulling on the wrench, it suddenly loosened, and his momentum caused him to hit the back of his head on a tire rim. Employee was knocked unconscious and taken to the hospital for observation. Tests were negative for any head injury.
