Miner was walking from SS to SCD, slipped and fell striking EE's head on the ground. The miner didn't lose consciousness and EE got up and went to SCD where EE reported the fall. Miner initially refused medical treatment then decided to go to the ER for evaluation. Physician removed miner from duty for 1 shift as part of a mild concussion treatment.

A SCD operator was replacing the drive belt on a conveyor when EE's gloved right hand became entangled in the sheave due to not performing lock out tag out. The miner sustained multiple injuries, including a fracture and multiple lacerations, to EE's right hand.

Employee pushed up safety glasses repeatedly, got some emulsion on their nose from gloves. EE developed small blisters later in the evening. Went to clinic two days later, diagnosed with 2nd degree burn.

Employee, with the help of coworker who was on the bed of a pickup truck, were unloading a 123 lbs. box from the pickup bed. Employee was holding the corner of the box with their right hand when the box was dropping off the truck. As the box was dropping to the ground, employee stopped the box from falling and was easing the box to the ground when felt pain in their right arm.

A forklift operator exited the forklift and attempted to disengage the catch lever manually. The employee placed left hand on the forks to stabilize the hopper skid plate. The employee disengaged the hopper's catch lever causing a weight transfer to the front. The hopper's rear skid lifted crushing the employee's index and middle fingers between the fork and skid.

Dozer(D10) operator was pushing material. Operator was preparing to push a ramp to get off from a shot. Operator observed material falling off from the highwall. Operator tried to back up but material hit the Dozer. The material shattered the cab door window and minor damage to the loader engine area. The operator drove the Dozer to a safe location before notifying EE' team leader.

Employee of 6 months was working on crusher feeder motor the employee removed the guard, and there was more guarding around it so, they had to stretch. Employee was on their knees reaching arm-length away to diagnose and repair the motor. They stayed in that position for approximately 20 minutes. The pain increased gradually, it turned mild after they stood up.

Miner Injured moving a 55-gallon oil drum on dolly. Drum was not set properly on dolly and miner adjusted drum. While adjusting drum right pointer finger was pinched between drum and dolly resulting in a hairline fracture and laceration. Gloves were worn.

Upon review determined 7K1 was not filed. EE was moving railcars as Groundman. Inertia generated by failed coupling caused trailing railcar to move creating gap between knuckles.EE adjusted knuckle then walked between knuckles verifying hand brake was set. As EE was walking sideways between the knuckles, the rail car returned to the location of the failed coupling, crushing the EE.

On Thursday Jan 11th, 2024, a mill employee of 2 years and 11 months of experience went to check on CBE 74 by the tunnels at 03:00am. When the employee entered the door to the drier building, a gush of high wind pushed the door getting their fourth finger on the right hand slammed between the door frame and the door.

A technician was performing EE's lube routes Sunday, 17-Dec-23 at approximately 2 p.m.. EE was carrying a bucket with EE's lube gun. While walking on the transition point, EE slipped and fell to EE's knees. EE attempted to break the fall with the arm carrying the bucket. EE felt a "pop" in EE's right shoulder.

While removing a product line on a railcar the ee pulled the line hitting a tooth, causing it to chip. Temp. agency reported this incident as a first aid. It was not until EHS manager spoke to the ee on the week of 2/5. EE mentioned the chipped tooth was repaired at the dentist. Since it was a temp employee from an agency, EHS contacted MSHA for advice on behalf of SMI.

On Wednesday October 4th, 2023. A swing shift employee of 2 years and 3 months in the company suffered a back sprain while descending from the G-loader at the beginning of their break at 22:30. As the employee stepped down the ladder, their left foot slipped and went through the last and 2nd to last rungs making them loose grip of their left hand from the handrail.

Two maintenance technician were replacing a rotary lock motor. As the motor was stuck a sledge hammer was used to tap loose the motor. However when one of the technicians struck the motor with the sledge hammer not realizing the other technician apparently had placed left hand under the motor to support it when left thumb was struck.

During the installation of a rotary valve to a hopper the injured EE lacerated left pointer finger from a pinch injury between the two flanges of the equipment being installed. The EE was reaching between the flanges to apply a gasket material when load swing occurred shearing the EE's finger between the two components.

While operator was pushing material, the dozer began to slide sideways away from the highwall. Eventually, leading the vehicle to slide down and roll over once and back upright. Immediate drug and alcohol testing was performed. Operator was seen by a physician. Physician did not find any injuries. Results returned positive for alcohol.

Kiln building location. Right eye injury: employee was wearing goggles in the kiln building. The goggles had some hot lime residue on them. EE removed goggles, and a small amount of hot lime entered EE's eye causing a burn.

The injured EE suffered a hand laceration while cutting a piece of steel (ball screw) while cutting the screw it snapped and cut the injured EE right hand resulting in stitches to the EE's right hand index finger knuckle

Employee was moving a waste hopper via forklift to a dumpster. They dismounted the forklift and placed a hand at the base of the hopper and then released the hopper latch. The hopper rolled over left hand, lacerating middle finger. Employee was taken to the hospital where two stitches were applied.

EE attempted to retract a magnet carriage into place. The carriage did not fully retract as prompted. EE performed LOTO and opened the hatch to the magnet to then find the magnet half closed. EE attempted to maneuver the carriage to return it to an open position simultaneously the cylinder actuated closing the carriage on EE's right hand. EE suffered a contusion.

A temp EE finished loading a 1000 lb. super sack when the EE noticed the super sack started to slide off the forklift forks. The EE thought they tilted the forks back. The EE then grabbed onto the super sack and felt pain in their lower back. On 2-15-22 temp EE was evaluated and given a prescription and a back belt for a lower lumbar strain.

EE was retrieving a piece of plastic hose that was below a catwalk on the roof of the building. EE used a broom to retrieve the hose. While pulling the hose over the guardrail of the catwalk towards self with right hand, frozen material shifted within the hose pulling EE's right wrist outward causing sharp pain. EE was diagnosed with a strained wrist.

EE reported on 12/6/2021 they felt a sharp pain in their upper back while carrying a bucket of tools up to Pebble Mill #2 blower. On 12/7/2021 EE requested to be evaluated and was diagnosed with a cervical strain; EE was given a prescription and put on modified duty.

A Scalehouse employee reported on EE's shift at 6:45am. The employee decided to take a break outside the scalehouse area, and leaned up against a metal chain connected to a trailer building and scalehouse structure. As EE leaned on the chain, the wood trailer structure gave way, causing the employee to fall on EE's back with the chain wrapped around EE's lower left leg and ankle.

A temporary haul truck driver (#54) was climbing up the steps to enter the drivers seat, when then opened the door (drivers side) to enter. The operator was holding the door handle, and as they opened the door, they struck the adjacent railing, and smashed right index finger into the railing, causing a non-displaced fracture.

The employee snipped the bands off a pallet located in the bed of a pickup with a wire snips and felt a twinge in their back. At first appointment on 12/5, the provider restricted the employee to desk work until an MRI was completed, but did not indicate it was work related. The neurologist provided a report on 2/5 that said it is work related.

Right index finger tip pinched between v belt and a sheave during a belt change. The injury was a fracture which needed surgical repair so the employee lost three workdays while getting medical attention. The employee returned to work on 8/12/19.

A Temporary employee was in process of loading an incoming truck, when the mast chain drive became loose by hitting a bump on the roadway. They decided to try and put the chain drive back in place by placing their left hand inside the chain pulley, when the pulley snapped back in place, catching the left finger, causing a fracture.

The Haul Truck Operator driving a 777D unit was traveling with a load to the Quarry Crusher, when the operator veered from roadway, & traveled through an approximate 5'7" berm. The truck rolled down an approximate 15-20 embankment & landed upright at the next roadway area. The operator allegedly nodded off & lost control of the haul truck.

Employee bent over to pick up an empty plastic bottle, 4 oz., that had dropped, and felt a sharp pain in the back. EE reported to the supervisor and went to an urgent care facility that took them out of work. Then EE went to preferred provider back specialist who said EE could work with restrictions and that the injury was not work related.

Truck driver having truck positioned onto above ground scale to load product into tank trailer. As driver exited truck cab and stepping onto scale railing (flat surface) EE held a 10 foot aluminum pole to position spout, lost footing and fell onto railing. Result was fractured rib. Doctor has reduced EE's duties to modified duty at desk after one day off.

While trying to unplug wet stone the 273 feed bin the employee reach into the bin cone to remove some stone that had the consistency of mud. The bin unclogged and dropped hot stone, freshly delivered from the SCD dryer, onto hand. Hot stone went inside the employee's glove and gave parts of hand 2nd degree burns.

A Quarry Tech was driving an empty haul truck down into the quarry along a haul road. When the front driver's side truck tire hit a ~ 4" deep depression in the road the truck gave the Quarry Tech a jolt which jarred neck causing a strain.

Employee was using a forklift and noticed that there was slack in one of the chains which raise and lower the forks and decided to adjust the chains. The Operator tried to realign the chain onto the pulley, when the chain moved and caught the right hand between the chain and the pulley. This resulted in avulsion to the right index finger

Mechanic was in the process of removing the sump lid to gain access to the sump by using an electric winch to move the 400 pound lid out of the way. Upon freeing the lid, it moved and caused left hand to be caught between the edge of the lid and an electrical panel mounted to the wall, resulting in a contusion type injury requiring stitches

The employee was removing the tin end of a condenser containing a calcium crown when employee's tooling slipped and employee hit employee's left forearm against the adjacent crown causing a laceration. The employee was not wearing forearm cut protection as required by standard work.
