They were repositioning the dredge in gusty winds when the starboard side positioning line became taught. EE attempted to ease the line out by pulling the dog back out of the sprocket teeth when ee did hand slipped off the winch handle causing it to free wheel the winch handle came around hitting right hand and ee nose as went down to the deck.

EE was operating a hand winch adjusting the dredge position. EE felt hand get weak followed by a sharp pain. EE was taken to a doctor where the hand was x-rayed and wrapped. EE was released to go back to work at normal job with restricted use with EE's right hand. EE was released back to full duty one week later.

Dredge operator was releasing the brake on the positioning winch, as the pressure was transferred to the winch handle he lost his grip and the winch handle came back around striking his hand. The supervisor took employee to the clinic and treated for swelling. On 04/16/14 employee was examined and it was determined that he has broken a bone in his index finger on right hand.

Employee was replacing the drive belt on the shaker of the rock washer and sizing plant. Some the rock had fallen from the shaker onto the walkway. Employee slipped on the rock and began to loose his balance, he then reached out and grabbed a brace on the shaker. His shoulder felt a little sore but seemed ok. The next morning his right shoulder was stiff and sore.

Employee suffered a stroke while preparing to leave work at the end of the day. He was found unconcious and unresponsive sitting in the Cat D7 dozer (by a Ready Mixed Concrete employee) where he had parked the dozer at the end of the shift.

The retaining clip slipped off of the winch causing the winch handle to spin free hitting the miner in the right hand fracturing his little finger.

EE was working from the catwalk attached to the sand separating unit on the aggregate gravity screening tower when the unit became out of balance and shifted knocking him over the guardrail causing him to fall approx 15 feet to the gravel pile below. He landed on his back causing bruises and a lacerated spleen.

The latch on a fiber rope winch released unexpectedly causing the winch handle to come around and strike the miner on the back of his left hand.

EMPLOYEE WENT TO RELEASE A HANDWINCH ON THE DREDGE, HIS LEFT HAND SLIPPED OFF OF THE WINCH AND THE MIDDLE FINGER BECAME CAUGHT BETWEEN THE GEARS IN THE WINCH.

EE WAS TIGHTENING A BOLT ONT HE SUCTION PIPELINE ON THE GRAVEL DREDGE WHEN THE COMBONATION WRENCH HE WAS USING SLIPPED OFF THE HEAD OF THE BOLT AND THE WRENCH STRUCK HIS FACE BELOW THE LEFT EY E CAUSING A LACERATION REQUIRING 3 STITCHES.
