Replacing Conveyor belt with the use of 262 D Cat Skid Loader driven by employee. Employee was backing the skid Loader while watching commands from two other employees when the conveyor clamp slipped off the belt and sling shot thru the windshield hitting skid loader operator in the right upper chest.

Digging out a indention hole with a hand held pry bar to hook the chain into. Felt a pop in right shoulder. 7/7/11 Ed told Ronda of the accident and he was going to the dr 7/8/11. Ed would need a MRI on his shoulder around the 19th of july. Ed has a torn rotator cuff and will need surgery. His next appt is Aug 4th for consult.
Employee claims he was using 3 point contact on the catwalk of the loader and was cleaning his window off in the rain when he slipped and lost grip due to the rain and fell to the ground injuring his wrist and arm.
EE tried to loosen a tight bolt on machine with wrench. Pulling down grip slipped & he fell forward enough to smack his lip on the end of wrench busting top lip some & the bottom lip wide open. He reported to his co workers what happened & came to office he was treated with IBprophen & Ice on the lip The Sec took him to Dr. He stitched 2 stiches & returned for day.
EE WAS OPERATING A SCRAPER BEING PUSHED BY ANOTHER SCRAPER TO LOAD HIM AND WAS JARRED BY THE IMPACT.
