TRUCK DRIVER DID NOT PUT SAFETY BAR UP OR BLOCK OFF DUMP BODY FROM COMING DOWN ON HIM. HE THEN GOT UP ON TRUCK UNDERNEATH BODY WHERE HE HAD NO BUSINESS. BEING WHICH RESULTED IN HIS DEATH. HE WAS TOLD IF HE HAD PROBLEM WITH TRUCK CALL MECHANICS FIRST.
