Employee was standing on a piece of metal that was being bend so it wouldn't move and with hands on the frame of the conveyor above. An employee struck the metal with a hammer causing employee # 1 to lose balance. Hand was holding on the conveyor frame which put weight on wrist (left) injuring it.

Miner mechanic was working on dozer repair. The mechanic lost balance when removing hose cover resulting in a fall onto the dozer tracks and then onto the ground. The injuries were to ribs and back.

Injured miner was using a pry bar to lift a bearing on a LOTO'd pump to remove shims under it. Lifted bearing and used fingers to remove shim, pry bar slipped dropping bearing, catching left index tip of finger cutting it off.

Contractor was assisting mine operator in making modifications to the primary wash plant which involved removal of water pipe. Contractor installed rigging to hold pipe contractor was going to cut. As contractor completed cut, pipe shifted in the rigging allowing it to swing striking contractor on left side.

Employee began experiencing discomfort. He left the property for the store for Rolaids. There the discomfort worsened. He stepped across the street to the fire station. They checked him out and took him to a hospital in Memphis. There it was discovered that he was suffering a heart attack.

EE was removing a jammed piece of concrete from the collector belt of the gravel in-feed bin. The concrete fell and crushed his left index finger against the angle iron frame of the bin causing a laceration.

He had attached a hook and rope to the bottom screen in order to pull it out through the top. The hook slipped and his right elbow struck against the frame of the screen deck. No expected impairment.

While working on the shaker deck, he had to remove a length of conveyor belting that had been placed there to protect the screen. In removing the belting he felt a pain in his back.

EE noticed that the shackle connecting the cable to the cutter head was not properly installed and proceeded to re-install the shackle. He was standing on the ground in front of the cutter head assembly, when the assembly dropped approximately four inches. One of the tines of the cutter head struck the front of his left leg, causing an abrasion and contusions.
