Contractor was assisting mine operator in making modifications to the primary wash plant which involved removal of water pipe. Contractor installed rigging to hold pipe contractor was going to cut. As contractor completed cut, pipe shifted in the rigging allowing it to swing striking contractor on left side.

He was spraying to kill weeds and felt something in his clothing. He knocked out a spider. Later, at home, he discovered he might have been bitten by the spider. He reported it on Monday, 6/23/2014.

Employee began experiencing discomfort. He left the property for the store for Rolaids. There the discomfort worsened. He stepped across the street to the fire station. They checked him out and took him to a hospital in Memphis. There it was discovered that he was suffering a heart attack.

Installing the last tooth on an excavator bucket with a sledge hammer. A sliver of steel from the tooth broke off striking him in the left leg.

Over-exertion in lifting objects

While installing a drive shaft in the bulldozer, he felt a pain in his back.

While placing the gravel chute it fell over and pinned his right hand against the handrail. Received sutures at the base of his thumb.

Removing a bearing from the housing by striking a hammer with another hammer. A sliver of metal from one hammer struck him in the abdomen.

Struck by falling object

While changing out a conveyor belt on the portable crusher, the roll of belting slipped off the supporting chain and fell against his leg, pinning his foot and ankle under him.
