Tech got into the back of the service truck to retrieve a hose. While stepping out of the truck, techs foot caught the tailgate of the truck bed and began to fall backwards. Techs hand slipped off the safety handle attached to the service bed and EE continued to fall backwards hitting the ground.

Working on primary crusher, removing belt clamp, belt clamp still under pressure, when the clamp came loose it pulled back knocking the ratchet from employee's right hand, wedging hand between belt clamp and support bar.

Employee came down after a few mins not knowing a pry bar was sticking up from the ground. Employee leaped off the second step and fell back (due to ground level) into pry bar and cutting inner thigh. Slipped into rectum, aprox. 8 inches, tearing large intestine, and puncturing gall bladder.

My employee was working on a slurry pump as a front end loader that was (operated by Alamo Concrete employee) carrying a full bucket of slurry material as it passed by pump material spilled out of bucket and material accidently landed on my employee. Employee was suppose to return to work on light duty but was sent home for the fact we have no light duty work

Was working outside, got hot, came in for lunch, drank a cold drink, causing gastric reaction and chest pain. Went to hosp, released on 10/1/15, employee instructed to contact cardiologist for a full release before returning to work, called MSHA when we believed to be heart attack, diagnosed with heartburn. Denver office advised us to make notes.

EMPLOYEE WAS GREASING HIS FRONT END LOADER WHEN HE LOST HIS BALANCE AND FELL BACKWARDS HITTING THE LOADER BUCKET. HE RECEIVED A LACERATION NEAR THE ANAL ENTRY AREA REQUIRING STITCHES TO CLOSE THE WOUND.
