After fueling loader, tripped on fuel hose and fell off steps of loader

stepping out of control room, walking down steps, miscalculated last step, fell on both knees w/ impact to right hip

Employee came down after a few mins not knowing a pry bar was sticking up from the ground. Employee leaped off the second step and fell back (due to ground level) into pry bar and cutting inner thigh. Slipped into rectum, aprox. 8 inches, tearing large intestine, and puncturing gall bladder.

My employee was working on a slurry pump as a front end loader that was (operated by Alamo Concrete employee) carrying a full bucket of slurry material as it passed by pump material spilled out of bucket and material accidently landed on my employee. Employee was suppose to return to work on light duty but was sent home for the fact we have no light duty work

Was working outside, got hot, came in for lunch, drank a cold drink, causing gastric reaction and chest pain. Went to hosp, released on 10/1/15, employee instructed to contact cardiologist for a full release before returning to work, called MSHA when we believed to be heart attack, diagnosed with heartburn. Denver office advised us to make notes.

while loosing the clamp of the conveyor belt, the clamp slipped with the weight of the conveyor belt, smashing hand against conveyor belt rails.

while climbing down the ladder on the loader, right shoulder popped loudly, felt sharp pain causing right shoulder strain

EMPLOYEE WAS WALKING DOWN HILL FROM THE BREAK ROOM AND SLIPPED AND TWISTED LEFT KNEE.

EMPLOYEE WAS LOADING TRUCKS WITH A FRONT END LOADER. LOADER HAD A FULL BUCKET OF 1/8"" MATERIAL WHEN THE BUCKET WENT DOWN HURTING OPERATOR'S LOWER BACK. EMPLOYEE RETURNED TO DUTY WITH RESTRICTIONS.

WHILE INSTALLING SCREEN, EMPLOYEE LOST HIS FOOTING AND TWISTED HIS LOWER BACK.

EMPLOYEE WAS REVERSING FROM FEEDER IN A FRONT END LOADER AND RAN OVER A LARGE ROCK WITH FRONT LEFT WHEEL. THE JOLT CAUSED A LUMBAR STRAIN.

EMPLOYEE RECEIVED A RIGHT WRIST TENOSYNOVITIS ON 10/21/2009. HE RETURNED TO WORK AT THIS TIME. HE HAD A REOCCURRENCE OF PAIN ON 01/25/2010. WAS SENT TO SPECIALIST ON 02/05/2010. EMPLOYEE W AS PUT ON RESTRICTED DUTY ON 02/05/2010 TO CONTINUE THRU 02/19/2010. FOLLOWUP EVALUATION ON 03/05/2010.

Right index finger was fractured while tighting bolt on a crusher. Swinging a 15 lb. sledge hammer and caught finger between handle and crusher bolt.

Employee was replacing a 3 feet section of mud pump pipe when material shifted causing injury.

FELL FROM LOADER WHILE DESCENDING LADDER. EMPLOYEE LOST HAND GRIP ON RAIL AND FELL TO THE GROUND.

Descending the hill at stone plant operating a front end loader, accidentally hit lower bucket lever, causing the bucket to dig into the ground, throwing operator into the steering wheel. Employee sustained contusion of the chest wall.

EE WAS ASSISTING IN INSTALLING THE STONE PLANT APRON FEEDER CHAIN ONTO THE DRIVE SPROKETS WHEN MANUEL TENSION WAS RELEASED FROM THE CHAIN, THE CHAIN FLEXED CLOSED CATCHING THE EE'S FINGERTIP T HUS CAUSING A DEEP CUT TO THE RIGHT INDEX FINGER.

WHILE PLACING A GUARD ON THE TAIL PULLEY OF THE LONG BELT, THE GUARD SLIPPED & CRUSHED L INDEX FINGER Y

EE FELT SOMETHING BLOW INTO HIS LEFT EYE.
