EE sustained a contusion to the right forearm and minor swelling when the drill rod fell from above striking EE after the yoke cotter pin broke connecting the mainline and the rod while laying the rod on the rod rack.

Employee twisted at the waist while lifting a HDPE pipe fusing machine and felt a dull burning pain in lower back. Employee was evaluated by a medical professional on June 6th and diagnosed with a lower back strain. The employee was placed on restricted duty from June 3rd to June 7th.

Employee was moving sandbags by SB-6 when he slipped and fell causing his left leg to bend underneath his body. NOTE: On Sept. 16, 2009 employee felt discomfort in his knee, went to physician and was removed from duty.
