(Employee claimed the injury took place on October 2, 2013, but didn't report it until July 14, 2014). Employee claimed he was opening trapdoor to access crane deck. While opening the door made of deck plate, it came down on his head, bending head over to left shoulder.
On 9/26/13 employee reported a lower back stain, he was checked out by a First Responder, who said he was ok, employee didn't want to be checked out at hospital and was released to return to work duties. Employee reported to hospital on 2/28/14 complaining of lower back pain (associated to 9/26/13 incident) he was checked out and prescribed medication and to RTW in 2 days (3/3/14)
Employee was using an etching tool when a metal fragment entered the eye. The item was removed and medication for the eye was prescribed. They received a full release to work.
employee walking to check densitometer, slipped in a hole, sustained injuries to back, hip and groin.
While sweeping the screen floor, employee bumped head against an overhead pipe which resulted in a minor neck strain resulting in lost days
Employee was shoveling ore out from under the primary conveyor. the crew was rotating 10 minute shifts and hydrating between. Coworker noticed signs of heat stress. Employee was transferred to med station for evaluation and taken to hospital for an exam. IV saline was administered
Individuals shoveling under primary crusher belt. Rotating 10-15 minute shifts with hydration between shifts. Individual was observed by co-workers to display signs of heat exhaustion, employee was given saline constituting a recordable illness.
Employee was performing house keeping duties near the primary feed when they presented signs of fatigue, they were given water and escorted to the first aid station where the decision was made to take them to the hospital. Individual was given IV fluids and released home.
Employee was stung by wasp while working
Individual opperating D9T, track slid into a depression, operator was shaken in his seat causing a neck strain
Employee was moving fencing material, and pulled a muscle in groin. Employee did not report the issue for several weeks following accident. Diagnosis identified a muscle strain in the abdomen/groin area.
Placing 3/4" socket set into a cabinet in lower tool room when they felt a slight pain in their back.
Employee slipped on a small rock while walking and sustained a slight injury to their left hamstring.
Injured person tried to move a full 55 gal oil drum and strained his back
The employee was injured on the 22nd of May and reported the accident on the 24 of May. The employee was taken to the clinic where a piece of metal was removed from his eye.
Employee stepping off of flatbed truck cab, noticed slight pain in shoulder. Later reported pain in shoulder and was assessed by EMT. employee returned to work and visited doctor on 22nd. Administered medication and restricted work.
Not securing the pin while trying to remove it from a load block for repairs. The pin fell to the ground smashing the employees finger.
Employee tripped while walking at the pit staging line. Employee received minor contusion on hand and received medication for discomfort in back
Individual was refueling a piece of mobile equipment when they stepped into a small depression in the dirt causing them to step and sustain an injury
Individual stepped into a pothole near a parking ditch and injured their leg.
A drilling operator sustained a minor back injury while installing a drill bit.
Employee lifted a piece of sheeting while assisting in securing a load to a fork lift when they noticed a pain in their lower back.
Employee was moving tertiary crusher bin on to bin 2. Switch bar loosened and fell onto employees right shoulder. No immediate injury observed. at 1:20am employee noticed pain while using wheel barrel. Escorted to hospital where diagnosed with contusion on right shoulder. Employee on restricted work.
Employee walking to parking lot after end of shift. Employee stumbled and fell onto left hand. Resulted in sprain to left wrist and a slight contusion on left hand.
Individual completing pre-op inspection was mounting a forklift. While maintaining three points of contact and stepping up into the cab, the employee felt a pop in the right elbow. Resulting injury initially diagnosed as tear in right distal biceps tendon.
Installing tooth on LD995 corner adapter, tooth slipped off and dropped approximately 4", injuring left middle and ring finger. Diagnosed with small fracture to fingertip, one fingernail removed by doctor. Fingers were bandaged and employee was released on light duty.
Tradesmen International Employee was walking into the regrind mill #1 and a chain link fence fell striking him in his left shoulder. He was diagnosed with a contusion of the left shoulder and was placed on restrictive duty. Employee will be attending 2-3 physical therapy sessions.
While removing the sheathing from some wire the tool cut threw the glove leaving a inch long cut to the palm.
During the process of pulling a core barrel out of a 60 degree angle hole, driller lowered the head and employee's left index finger was pinched between the head and rail on the drill mast resulting in an open fracture to the tip of employee's finger.
While hammering wedges with a 12 pound hammer the employee felt a sharp pain in his right shoulder.
Employee was sliding tube onto rod truck and finger was smashed between tube and rod on truck resulting in minor fracture to tip of finger and torn fingernail.
Fatality due to fall from heights.
while removing a dust mask from his face a strap from the mask snapped and coming back and striking ee in the eye. this caused irritation and redness to the effected area
While working at the Heavy Equipment staging area, employee received a bug bite on lower left side of chin. Not exactly sure when it happened. Received medical treatment on Sept. 14th and was released the same day for regular work activity.
Employee was bitten on hand by spider after picking up gloves.
During grinding an employee stopped to wipe sweat from his forehead and eyes. Afterwards he began to feel irritation in his left eye. He proceeded with eyewash.After 15 minutes of flushing irritation was still present so he was driving to a medical center to have sliver of metal removed from eye. He was released to full duty afterword and reported back to work that afternoon
Employee stepped out of truck and slipped on loose ground.
While deploying liner material on the pond floor a gust of wind reached our work area. The gust lifted the liner material the IE was standing on the liner at the time the wind hit our location. The EE fell to the ground.
While doing the copper harvest, returning starter sheets to the cell, stepped onto cell and foot slipped causing a fall into the cell with one leg which hit the lead anode in the cell at his knee.
Employee was dismounting forklift and hand slipped off the handrail striking body of forklift. Dislocation and slight fracture of left fifth finger.
Doing regular routine work and later noticed my right eye was irritated and very red and felt like something was in it while working at SX Plant.

Employee was positioning idler on pallet and got left pinky finger caught between the idler and the pallet causing the end to be cut off.
Employee was driving a pin out on a loader bucket when a piece of metal flew off and hit left ear. On 11-28-08 he asked to see doctor as ear was sore. X-ray determined that a 2mm piece of metal is lodged in the outer edge of left ear. He has not decided to have the piece of metal removed as of 12-08-08
Laceration to finger
While attempting to walk around some water walking back from crusher employee slipped and fell onto right knee and then while attempting to stand up fell a second time on left shoulder.
While cleaning out tail area of feeder with water hose, materials sprayed back and some got into left eye.
