Employee was helping to dig out a conveyor when they walked around the skid steer and stepped on the skid steer bucket when they slipped and fell on the bucket causing their hard hat to fall off and hitting their head on the bucket bolt. This caused a laceration on the top of their head requiring 15 staples.

Showing all 4On Friday, June 28th EE was walking to the value to fill up the water truck. EE started to walk alongside the tanks and EE's right leg slipped out in front of EE causing EE's knee to twist making EE to fall backwards. EE injured EE's right knee and dislocated EE's patella.
Material in high wall fell on Dozer entrapping operator.
While lifting the cone liner approx. 3', employee was standing at the side of the liner checking the bottom rim when the off-side weld broke on the lifting eye shifting the liner knocking employee to a knee and the other weld broke causing the liner to fall on the employee's right knee and right forearm.
