Employee was using a sledge hammer to try to free the wear plate on a D10 dozer blade. A metal fragment broke off the plate and struck the employee in the leg.

Employee was operating a Haul truck when EE went over a mound of material at approximately 30mph. EE received friction burns and a lower back strain.

The contract truck driver working for STM reported slipping off the last step of the truck and falling to the ground when in the process of trimming the load. EE reportedly put hands out to catch self. EE left the site after falling and sought medical attention.

EE was walking under Conveyor PC-12 and slipped when they stepped on a rock. Their right knee jammed when their foot slipped. The EE was offered medical attention and refused at that time. The EE reportedly twisted their knee over the weekend when their foot got stuck in the mud. EE reported twisted knee to their Lead Person on Tuesday 3/24 requesting medical treatment.

Employee was pulling a pin out of the ground and felt a pain in their lower back while doing so. On August 3rd, the employee made a determination that the continued pain warranted being evaluated by a doctor. The visit to the doctor occurred on 8/3/2018.

During the pre shift inspection on the Dredge EE hit the (top) back side of their hand and EE does not recall what they hit their hand on but does recall while inspecting a dewatering screen they hit their hand on something. At the time they did not notice anything but about a hour later they noticed their hand bleeding.

EE was climbing into screen #701 to replace skirt rubber that deflects rock at the end of screen. While entering the screen EE had to push existing skirt. While pushing EE heard a "pop" in their back but did not feel pain so continued & completed the task. In the next morning while getting up EE's back was sore & EE notified supervisor (mill foreman).

EE was assisting with removal of the front hubs on the 988F front end loader. While they were attempting to place the hub on shop floor with use of overhead crane, EE had placed a lifting strap around hub & to the crane hook, EE was trying to force the hub to set down level & right hand middle finger became pinched between strap & a 90 oil fitting on the hub.

Injured miner received bee stings when a nearby dozer ran over a box containing a bee hive. The hive swarmed around the injured miner and stung EE on head and neck. EE was treated at the company clinic and released to full duty.

While assembling a 12-inch pipe, using an excavator as a crane, the pipe moved bumping EE in the right thigh. EE was taken to the clinic and diagnosed with a bruised right hip and strain of right thigh. Physical therapy was recommended. This was treated as a first aid until 6/15/2016 when the clinic requested that the EE see a doctor for continued pain.

A miner suffered from neck strain when the bed of his haul truck came down suddenly. The bed's hydraulic ram hose ruptured as it was descending causing it to fall. He was given OTC meds and restricted work.

After work a miner noticed he had a bump below his abdominal area. He noted that he had been shoveling material that day for a long period of time. He reported it 2 days later after noticing the bump became larger. He did not have pain but wanted to report it anyway. He was sent to the Industrial clinic after he reported it to management.

Loader operator was injured while driving loader on uneven grad the loader rocked from side to side and his right leg below the hip hit the metal bar support to the arm rest inside the loader. "This incident was just viewed as a first aid until a drain was applied to the bruise to aid in healing of the bruise."

Employee received a hypertextension injury to his left thumb while clearing spilled material adjacent to a shaker screen. The employee was using a shovel to clear rocks that had spilled from the 711 screen onto the work deck. His foot slipped on a rock causing him to slip and fall which resulted in the shovel handle jam into his left thumb.

Employee complained of angina after shoveling and believed he was experiencing a heart attack. He was taken for med treatment & admitted to hospital. Tests indicated he did not suffer a heart attack. It was described as an "episode" related to childhood heart surgery. Employee related that he has these episodes periodically, not work related. MSHA was notified as a precaution.

Injured employee suffered a smashing injury to tip of his right forefinger. Employee was engaged in removing the cab from an old Terex haul truck by cutting it away with a torch. As he finished cutting the truck's frame, the cab settled, nipping the end of his right forefinger. The injury resulted in a laceration requiring sutures to close.

Employee reported on 12/07 that on 12/06 he believed he injured his lower back while performing a crusher liner change. He refused medical treatment since he wasn't sure if it was industrially related or not. On 12/10 he reported that he wanted to file a W/C claim. He indicated that the injury was from over-exertion. Provided pain meds, temp work restrictions.

Employee was operating a forklift near a bulk Dust Foam tank when he ran over a PVC valve on the tank, breaking it. Some of the tank's contents began to leak which he attempted stop by using a rag to plug the valve's broken pipe. As he reached down to plug the leak, he cut his finger on a jagged piece of broken PVC pipe.

Employee was installing rubber screens with a slide hammer in the middle deck of a screen. When he was hitting one screen in the bottom of the hammer slipped off the panel and causing the whole slide hammer to fall suddenly and smash his finger between the handle of the hammer and the bucker bar rail on the top deck.

Employee was removing ram cylinder from 773D. Chain hoist was attached to cylinder with nylon strap at center of ram. When employee pulled on the lower clevis with his hands, the ram came free and swung towards fuel tank smashing his hand in between resulting in pressure cut to middle finger requiring stitches.

Prior to shift, victim was walking from his personal vehicle to the office. Another employee driving personal vehicle did not see victim and struck him, shattering pelvis and breaking hip. Conditions were predawn with unprecedented fog, visibility 25-50 feet.

Employee was working with others repairing a crusher. As the employee was walking on the catwalk, he stepped on an uneven surface and twisted his ankle, causing him to fall. Employee was diagnosed with an avulsion fracture and placed on modified duty.

A welding crew was working on the middle of three stacked screens, which had some rubber/urethane screens. The crew failed to mitigate the fire hazard posed by the hot work near/above flammable materials. During welding, slag or other hot debris fell to the lower screen deck, setting it on fire. The flames were out in less than 30 minutes. There were no injuries.

Employee was overhead welding a cross brace in the wet screen when hot slag dripped into the gap of his long sleeve shirt where it buttons at the wrist and it gave him 2 small burns on his left elbow. The small burns developed into an infection which resulted in lost time because employee did not report the incident to his supervisor until 4/14/2009
