Employee suffered a right calf strain when he stumbled handling a hose to wash mud from his haul truck. He did not feel it needed medical attention until 9/24/2014. He was treated for the strain and given modified duties until 9/29/2014.

Employee complained of angina after shoveling and believed he was experiencing a heart attack. He was taken for med treatment & admitted to hospital. Tests indicated he did not suffer a heart attack. It was described as an ""episode"" related to childhood heart surgery. Employee related that he has these episodes periodically, not work related. MSHA was notified as a precaution.

Employee was operating a forklift near a bulk Dust Foam tank when he ran over a PVC valve on the tank, breaking it. Some of the tank's contents began to leak which he attempted stop by using a rag to plug the valve's broken pipe. As he reached down to plug the leak, he cut his finger on a jagged piece of broken PVC pipe.

Prior to shift, victim was walking from his personal vehicle to the office. Another employee driving personal vehicle did not see victim and struck him, shattering pelvis and breaking hip. Conditions were predawn with unprecedented fog, visibility 25-50 feet.

Slipped and tripped on soft, muddy ground, straining knee. Medical treatment obtained on Monday, 12/13/10, at which time work restrictions were given by doctor.
Employee was replacing a screen cloth on a screen deck in the Tertiary tower. The screen was difficult to remove and the employee pulled at the screen vigorously causing strain to his upper back.
Employee was working with others repairing a crusher. As the employee was walking on the catwalk, he stepped on an uneven surface and twisted his ankle, causing him to fall. Employee was diagnosed with an avulsion fracture and placed on modified duty.
Employee was assisting with repairs in the Secondary MCC room that required crawling. The concrete caused injury to his right knee, that resulted in swelling.
Employee was entering the crusher hopper to begin replacing liners. Employee lost his footing on loose material lying on the belt and fell onto the liner wall striking it with his right shoulder.
A welding crew was working on the middle of three stacked screens, which had some rubber/urethane screens. The crew failed to mitigate the fire hazard posed by the hot work near/above flammable materials. During welding, slag or other hot debris fell to the lower screen deck, setting it on fire. The flames were out in less than 30 minutes. There were no injuries.

The employee was steadying the crusher distributor head as the crane was lowering to the crusher deck. As the crusher head leaned over, the chain slipped on the lifting eye pinching the employee's middle left finger between the lifting eye and the chain links. Laceration to finger requiring stitches, no lost time.

Employee was assigned to clean & repair reclaim belts counter weight guards in the Tertiary plant. While the employee was attempting to reinstall one of the panels, the panel came off the hooks on the opposite side smashing the employee's right hand little finger between the post & panel. Injury was not reported to supervisor until 7/2/08.

While carrying a 1 1/8' x 4' x 8' wire screen cloth in the tertiary tower the employee felt a strain to the upper back.
Employee was adjusting the 4 1/4 cone crusher with co-worker. After taking up the crusher by running some rock through, the workers began to re-set wedges. One of the posts started to drop. Employee grabbed the post with his left hand while co-worker shoved a wedge into its slot catching the employee's finger between the inside of the slot and the wedge.
While using a 12 lbs hammer to clear rocks form a vibrating grizzly screen, the employee struck the right index finger with the hammer.
While transporting a manlift from the concrete batch plant to the primary rock plant the employee hit a curb. When the left front tire hit the curb the employee was thrown forward hitting his lower ribs on his left side on the manbasket handle.

Employee went to put his welding helmet when a foreign body fell into his left eye.
While pulling 1"" rope to tie onto a conveyor belt, the employee felt pain in his left shoulder.
Dumping fill dirt on grade, End-Dump trailer tipped over attached with Semi Tractor.
The employee was climbing down from the forklift he had been operating. As he stepped onto a rock, he lost his balance. In trying to catch himself from falling, he felt his right knee pop.

The employee was hammering a keeper pin in a new ripper shank guard on D11 dozer. He steadyed the guard with his legs while hammering, striking his left knee with the hammer.
While cleaning out a reclaimer drain, the drain sprung a leak. The cementitious water from the leak sprayed the employee in the face, dripping down into the right eye.

ROCK STRUCK EE WHEN LOADER OPERATOR DROPPED LOAD INTO FEEDER. EXCESS ROCK WENT OVER FEED WALL WHILE EE WAS DOING DAILY GREASING. INITIAL RESULTS WERE FOR EE TO RETURN TO WORK FULL DUTY. ON 4/29/03, EE HAD NOT IMPROVED. FOLLOW-UP MEDICAL TREATMENT PRESCRIBED MEDICATION AND TIME OFF WORK.

THE EE WAS PULLING A TAILPULLEY GUARD FROM THE BACK OF A SERVICE TRUCK. THE DOOR SLAMMED SHUT ON THE EMPLOYEE'S FINGERS OF HIS RIGHT HAND.
EMPLOYEE WAS CLIMBING DOWN THE LOADER LADDER AFTER LOWERING THE BUCKET TO THE GROUND. ABOUT HALF WAY DOWN, THE LADDER, HIS HAND SLIPPED OFF THE HANDRAIL WHICH CAUSED HIM TO FALL APPROX 4 FEET TO THE CONCRETE SLAB.
