EE WAS STACKING PALLETS WHILE LOADING A BULK TRUCK AND TWISTED HIS BACK. HERNIATED DISC IN BACK.
EMPLOYEE COMPLAINED OF SORE RIGHT ARM AFTER STACKING 100# BAGS ON PALLETS. REFUSED TO GO TO DOCTOR. ON 3-19-01, HE LEFT WORK MID DAY AND RETURNED WITH DOCTOR'S NOTE RECOMMENDING NO HEAVY LIFTI NG UNTIL 4-5-01. HAS NOT RETURNED YET.
