EE began feeling ill while driving EE's haul truck. Quarry Manager was notified and took EE to the hospital in Manager's truck, where EE was observed and then released later the same day. Results are inconclusive.

Showing all 34Uneven ground, smooth rock. Poor footing. Worker slipped on packing material from metal the worker was transferring to pallet. Packing material covered a rock and worker fell and hurt left knee.
The production crew on shift was clearing a pre-heater tower plug in the feed pipe and stage 4. EE inserted Cardox tube into port and while trying to set the depth of the Cardox tube they felt pain in lower back on the left side. EE initially decided on self care however requested evaluation on 12/20/21 and was released to work with restrictions.
Employee was in the process of cleaning the laydown yard. Part of the clean up involved removing some steel plate to take to the scrap pile. As the employee was setting the piece of plate into the bucket of a skid steer, EE felt a sudden pain in the left bicep.
EE entered a passenger elevator, closed scissors gate, the exterior door did not fully close, EE reached through scissors gate to secure exterior door and smashed right thumb between door and frame.
On Sept 15, 2021 EE preforming electrician duties was pulling cable through conduit, tweaked EE's right shoulder while wiring a speed switch. EE refused medical attention, incident documented. On Oct 20, 2021, EE requesting a medical evaluation of the shoulder. Medical evaluation dictated restrictions, returned to work immediately.
The mine operator was made aware of a STS shift in a miner's hearing. The miner has been in the hearing conservation program and effectively uses hearing protection during performance of EE's duties at the mine.
Employee was in the process of removing sand that had accumulated under a conveyor with a bobcat. The employee had made several passes under the conveyor until ee struck a vertical ladder with the bobcat attachment. The bobcat struck a vertical ladder and a 6 inch piece of rebar flew off the crusher deck and struck the operator on the forehead causing 1-inch laceration.
Employee was using a locking blade utility knife to cut a section of urethane. The employee cut into the material and the utility knife got stuck. The employee started to twist the knife and extra force to cut the material. The knife slipped and the employee was not able to stop the knife before it struck the employee's arm causing a laceration to wrist area.
While working on Simons Crusher the miner was loosening the bolts to the crusher spring pack when the bottle jack supporting the spring pack bottom plate displaced out towards the miner from the bottom of the spring pack plate striking the minor on the left forearm.
Unknown rock or debris falling down chute into work area. Striking Employee in Genitals area causing a contusion.
Miner/Employee was assisting in replacing new sand screw wear shoes when EE was repositioning the shoe to the shaft it slipped from his grasp and resulting in a caught between the sand screw tank wall and screw auger flight crushing/minor fracture injury to his left index finger.
The employee was cleaning up the surrounding work area when he went to pick up a piece of steel pipe he strained his back.
Employee was cleaning out around conveyor tail pulley and stepped into a muddy area and stepped into a muddy hole.
Employee was climbing into chute area of prewash plant via access door then slipped and fell backwards striking a rock in the chute straining lower back.
TWISTED KNEE WHEN STEPPING OFF EXCAVATOR ON UNEVEN GROUND.
EMPLOYEE WAS TRYING TO RELIEVE PRESSURE ON A HYDRAULIC SYSTEM ON THE AUXILARY CRUSHER BY TAKING THE FITTING OFF A CYLINDER. EMPLOYEE DID NOT VERIFY THAT ALL THE PRESSURE WAS EXHAUSTED FROM THE HYDRAULIC SYSTEM. WHEN THE EMPLOYEE ATTEMPTED TO REMOVE A HYDRAULIC HOSE THE COUPLING KICKED BACK AND CAUSED AN INJURY TO THE EMPLOYEE'S RIGHT HAND.
Employee was lifting large batteries and strained left groin.
Employee was working on a 988G loader bucket cutting edge using impact wrench to remove bolts. Bolts spun and employee tried to hold the bolt by hand to lock it in place and bolt spun cutting his left middle finger through his glove.
WATER TRUCK DRIVER WAS FILLING TRUCK WITH WATER, REMOVED ROPE THAT HOLDS VALVE OPEN & ROD CONNECTED TO VALVE CAME DOWN AND HIT THE DRIVER ON THE HEAD. HEAD CONTUSION.
EE WAS KNOCKING DOWN BELLY DUMP WINDROWS WITH LOADER. HE HIT A WET SPOT AND SUNK TO THE RIGHT SIDE, CAUSING EE TO TWIST AND STRAIN A MUSCLE ON RIGHT SIDE OF LOWER BACK. EE WAS IN SEAT AND HAD SAFETY BELT ON.
EMPLOYEE WAS GRINDING/WELDING ON THE LOADER BUCKET. HE NOTICED NOTHING OUT OF THE ORDINARY DURING WORK SHIFT BUT WHEN HE WENT HOME FELT A DISCOMFORT IN HIS LEFT EYE (FOREIGN BODY/FLASH BURN).
EE WAS REPLACING ELECTRICAL PANEL WHEN THE PANEL SLIPPED OUT OF ITS SAFETY CHAIN HARNESS & FELL ON EE'S LEFT HAND RESULTING IN A LACERATION.
EE WAS REACHING INTO CHUTE OF ROCK PLANT AND STRAINED HIS LEFT ELBOW & FOREARM WHEN PULLING BACK SKIRT.
EE WAS AT BUNKER TO START JOB AND STEPPED OUT OF TRUCK TO FILL TRUCK AND GRABBED I BEAM TO STEP OUT AND INSIDE OF I BEAM A PIECE OF METAL SPIN CUT LEFT PALM OF EE. EE SUFFERED LACERATION TO LE FT PALM.
WALKING ON CATWALK GOING TO CONTROL ROOM SLIPPED OFF CATWALK AND FELL AGAINST HANDRAIL.
EMPLOYEE AND CREW WORKING ON CONVEYER MOTOR BELTS AFTER CHECKING BELTS ALIGNMENT, CREW WENT TO CHECK TENSION. EMPLOYEE HAD HOLD OF BELTS AND ROTATED RIGHT RING FINGER AND LITTLE FINGER INTO BELT PULLEY.
EE WAS SWINGING A SUSPENDED BEATER-BAR WHICH BEATS THE UNDERSIDE OF THE GRIZZLEY TO VIBRATE & KEEP MATERIAL FLOWING FROM HOPPER. MINIMAL EFFORT IS REQUIRED TO SWING THE SUSPENDED BAR/HAMMER. E E THREW OUT RIGHT SHOULDER WHEN SWINGING BAR. RIGHT SHOULDER STRAIN.
STEPPED OFF SCALPING SCREEN ONTO WALKWAY AND TWISTED LEFT KNEE. HAD CLEANED WALKWAY PRIOR TO GOING ONTO SCREEN.

EE HAD EARLIER PERFORMED HEAVY PIPE WRENCHING INA CONFINED POSITION THEN HAD GROWING PAIN IN ABDOMEN AS HE CHANGED SCREEN CLOTH.
