EE was cutting steel in the weld shop with an oxygen acetylene torch when EE realized the oxygen hose was leaking from a hole. EE found where the leak was coming from and went to pinch the hose so EE could shut the gas tank off. As EE pinched the hose, both hoses exploded and ignited sending out a flame that contacted the EE's abdomen.

A millwright employee was installing the bracket for the pneumatic gate air actuator for Silo 1 bin bottom while using a hammer to knock the bracket into place. As the hammer was in swinging motion it hit the employee's left thumb as it was in a downward motion to knock the bracket into place.

While winterizing the lime slurry system an EE disconnected the line to clean it out when the system was offline. When the EE disconnected the line, a loose coupling caused the line to come loose prematurely. Residual lime slurry in the hose sprayed onto the EE getting on EE's face. EE immediately flushed EE's eyes with clean water, the burning continued and was brought to ER.

An employee was working on removing a bearing cap to inspect the bearing on "B Elevator" when EE came into contact with a damaged metal seal on the side of the bearing head shaft. While removing a piece of the damaged metal, a sharp edge cut through EE's heavy duty glove into EE's left pinky finger resulting in 4 stitches.

The employee had finished repairs to the Preblend Stammler system immediately prior to the injury, which occurred when employee was walking down the stairs with tools/equipment. The employee stepped down from the top step with employee's right leg, and as soon as employee put pressure on employee's leg employee felt employee's knee "pop".

The employee complained of wrist and finger pain. Their main role is administrative, and is part of the sales team. It is unknown how the injury/illness occurred, as the employee believes this is a chronic condition from using a keyboard and mouse.

The employee was troubleshooting the flop gate control arm on a FK pump. The employee's finger became pinched between the pivot arm and flop gate, causing a laceration on their right hand (5th finger).

EE was working on the 1812.2 Clinker Silo Dust collector fan. EE released the button and as the motor was winding down EE went to see if it had normal air flow. On the way by, EE's fingers came in contact with the moving belts. The tip of one finger was removed and the other was injured. EE reported to the control room and was then taken to the hospital for treatment

While moving a drum of air compressor oil using a hand truck, employee felt a twinge in the left side groin area. Employee kept on working. Later in the morning pain developed and a bump appeared in the same area. Note: injury IS NOW REPORTABLE DUE TO SCHEDULED SURGERY ON 11/16/16.

The Fane truck driver was unloading a trailer full of clinker. As he was raising the 30' box it became unstable at half it's height position and tipped both box and truck onto the driver's side. NOTE: LEHIGH NORTHEAST CEMENT CO WAS NOT AWARE, UNTIL RECENTLY, THAT THIS INCIDENT RESULTED IN INJURIES THAT WOULD HAVE MADE THIS MSHA REPORTABLE.

The Employee was helping to install new gudgeon bolts on the 1143 screw. While doing so it is necessary to use the drive belts and pulleys to align the screw for bolt insertion. In the process of doing this the screw backlashed and employee's right hand pinky finger was caught between the belt and pulley tearing a gash in the tip, requiring stitches.

The employee walked into an area that was inundated with dust to try and put a 4" cap back on to the pant leg of silo 29N. The force of the material coming through a 4" opening knocked off his dust mask and safety glasses, causing a scratch to his left eye and difficulty breathing.

The driver was lifting the batching station platform using the safety rail; his finger was pinched between the safety bar and the platform beam causing fractured finger requiring stitches.

The employee placed his right foot on the top rung of the ladder to dismount the gypsum feeder platform. When doing so his foot slipped off the ladder causing him to lose his balance and fall approximately four feet to the ground, causing right ankle sprain.

Employee was working in slag dryer DC fan shroud with concurved base, as he was inside he threw out a piece of steel ring he had cut from the fan plenum. As he threw object employee slipped on the incline in the fan housing and fell against the housing hitting his right side; shoulder, ribs, arm and a small laceration under his right eye.

The employee was working in a warm location and sweat was running down his face into his eyes. He wiped his face off to remove the sweat. He went home at end of shift but it did not seem to be bothering him. He woke up early in the morning with pain in his eye. We took him to an eye doctor the next morning. They found a tiny piece of steel and removed it.

Lifting corner of bridge crane with 50 ton hydraulic cylinder (port-a-power) to remove cribbing from under crane. As the crane was being lifted, the cylinder was ejected from under the crane and hit employee in chest. Contusion to chest, heart, lungs, spleen, laceration to liver and broken rib(s).

The employee was responding to a call-in, while walking through the roller mill building employee stepped on a medium sized stone, slipped and extended his left leg. Employee felt a twinge in his left knee and upper left thigh, but no pain until the next morning. Note: Employee did not seek medical attention until 11/6/13 and first lost day is due to surgery on 12/10/13.

The employee indicates that over the last couple of years he has been noticing numbness in the fingers of both hands, with the most pronounced numbness being in the fingers of his right hand. This date of 4/25/11 is the first time the employer was made aware of this condition. We are now reporting it as a reportable, as surgery has been scheduled for July 1, 2011.

While traversing the steel storage area, the employee struck a sheet of steel and fell. This caused a laceration and bruise to the right knee. Doctor visit of 6/1/11 was the first time the employee sought medical attention for this injury.

While moving impact plates from catwalk with the hoist, other impact plates stacked for removal tipped over and struck employee in right calf and slid down the back of employee's leg pinning his foot and ankle under the plates, causing a contusion/strain injury.

While driving out liner bolts the bit of the jackhammer became wedged in the hole opening causing the hammer to pull in towards the shell of the mill. This action caused the employee's hand to become pinched between a mill rail and the trigger of the hammer. This caused severe pinching of all four fingers on left hand.

The employee was installing electrical cables into an electrical tray when he was struck by a ragged piece of limestone approx. 4"x3"x2" that fell from the 957 conveyor belt approx. 45'1. This caused an abrasion and swelling to his left side just above his waist.
