69 caps and boosters were shown on the blast report for the blast on 9/27/2018. After investigation, the blasting contractor determined that 68 detonated during the 9/27/2018 shot.

Stepped back into small hole on Shaker Screen and twisted Left Ankle.

Employee was repairing water line on the south part of the property. Employee took short cut through ditch line, on state property, when repairs were finished. While descending embankment in ditch, employee loosened material and fell causing him to fracture his lower left leg. The ditch separates mine property and state property.

Employee was using a wire brush in a hand drill with safety glasses on and got something in his right eye. Employee was sent to eye doctor and no material was found in eye but was given prescription antibiotics with no restricted duty.

LIFTING MACK ENGINE WITH CHAIN HOOKED TO 988-B LOADER. LOADER OPERATOR SET ENGINE ON GROUND. EE UNHOOKED CHAIN & TRIED TO BALANCE WITH HIS LEG & COULDN'T HOLD IT UP. ENGINE ROLLED OVER PINNING HIM UNDERNEATH IT. RESULTING IN INJURY TO LEFT LEG AND LEFT SHOULDER.

RAIL WAS OFF SCRREN AND EE GOT OFF LOADER TO REPLACE RAIL. EE LIFTED RAIL AND STRAINED BACK.
