Replacing a coarse material washer (CMW) located inside a tub the employee was pinned between the CMW and a material chute above the tub. the employee was pinned for approximately 30 seconds, briefly lost consciousness and suffered bruising to the ribs and chest.

Laceration to the non-dominant hand, Right ring finger approx 1/2 inch in length. 7 stitches. Employee was unbolting a screen deck with a leather glove and battery operated impact tool. As EE was removing the bolt, the bolt spun and it had a small metal bur on the end. Which spun and cut the glove and the employee's finger on the distal digit (tip) of finger.

Knife River miners were replacing a screen. Our sales representative was on site. He was on site to provide technical assistance. He was struck in the head by the feeder box. EMS was notified and responded. A Knife River operator notified MSHA within 15 minutes. Injured person was taken to the hospital where he then passed away.

Two men were replacing bottom seal ring on cone crusher. The seal ring slipped off retaining lip while installing bolts mashing man's left hand 2nd finger between ring and main shaft.

Reportable hearing loss 1-16-06 had sinus infection after repeted doctor visits, M.R.I, Cat Scan, Full audio Tests, on 6-28-06 doctor was unable to rule out noise induced hearing loss, damage may have been as mutch as 10 year ago. hearing loss in both ears in exess of 10 db
