Employee was attempting to clear a primary screen discharge box that was clogged with returned concrete. Clog material was over engaged causing the employee to slip and strike an angle iron brace with hand resulting in a closed fracture of right pinkie finger. Employee returned to work at regular position but, with restrictions on use of right hand.

A 4' pry bar fell four feet from a conveyor belt and struck the top of the employee's foot just above where the protection from his steel toed boots ended, causing a fracture.
EE operating water truck drove over pot hole. The steering wheel turned to the right quickly with left hand gripped to steering wheel, causing shoulder strain & cervical strain.
Employee said that while exiting the cab of the 769 haul truck he bumped his right elbow on the arm rest. The next day 4/16/09 he noticed minor swelling so he was sent to the doctor for an evaluation.
Injured employee was in the process of fueling a Caterpillar D-9R Dozer when he slipped and lost his footing. The employee grabbed onto the dozer with his right hand supporting all of his weight with his right arm. The result was a right bicep tendon rupture, return to work with modified duty. No lifting over 5 lbs with the right arm may require surgery pending medical evaluations.
Employee struck his right knee on the lower corner edge of the driver side door of the truck.
Injured employee was replacing troughing rollers on the utility sand conveyor and pulled a muscle in his right shoulder while tightening the bolts with a wrench. 1/6/09 the employee reported that his personal doctor suggested surgery and that the injury apears to be work related. A claim was submitted and shoulder surgery is scheduled for 5/4/2009.
Employee stated that he hit a bump while driving a haul truck and jarred his back.
Employee was conducting routine maintenance at the end of his shift. Employee stepped up on a concrete block to adjust a hose used for dust suppression on a conveyor belt. As he stepped down his wedding ring was caught in the guard (1 inch square screen material) and the employee recieved an avulsion to his left hand ring finger
Employee took a shortcut after finishing work on the ABC crusher screen and, ignoring company policy, he climbed over a handrail stepping onto a moving conveyor belt running underneath the crusher screen. The belt was not part of the crusher screen that was being worked on. First aid was administered by an off-site provider on an out-patient basis.
Employee failed to wear a seatbelt, and buried bucket into the ground of his front end loader, causing employee to hit windshield and break thru windshield
Employee is a loader operator who was going to flip the teeth on the loader bucket. Prior to begining work, the employee was wearing safety glasses, but removed them to look for a hammer in the toolbox. The employee did not put his safety glasses back on prior to commencing work. The employee hit one of the teeth and a metal sliver flew into his right eye.
Shortly after clearing a rock from the Jaw Crusher, Employee had pain in his chest and collapsed from a heart attack.
While working on the Wash plant changing out the deck screen frame, the employee cut the welds holding up screen deck causing screen deck to fall onto right arm/elbow.
Employee was working on a shaker screen and was pulling on a pry bar to install a band on a motor when the pry bar slipped and hit employee in the mouth knocking a tooth loose. Dentist eventually pulled tooth in November
Injured employee was unplugging chutes in #2 wash plant using a 3/4" by 60" round stock. The chutes were shoulder height or slightly higher and while hitting chute, employee used force to dislodge material and injured right shoulder. (Torn rotator cuff; surgery performed 7/14/05.)
IE was climbing down the ladder on the jaw feeder when his foot slipped on the ladder and his leg went thru the ladder and he fell backward. The IE suffered a groin muscle pull.
The employee placed a ladder against a conveyor with the top into the framework. When he was climbing the ladder to tie it off it slipped out from underneath him. He was 3/4 of the way up the ladder when it slipped & he fell approx. 8' to the concrete pad suffering a compound fracture to his arm, contusions to his ribs & a fracture in his foot.
While the IE was using a grinder a peice metal came in behind his safety glasses and sheild.

WHILE IN THE JAY-JOHN A FELLOW EMPLOYEE DUMPED A BUCKET OF WATER AND SAND ON JOHN. THE EE SUFFERED A CHEST WALL STRAIN. ALL PARTIES INVOLVED WERE THEN DISCIPLINED FOR HORSEPLAY.
EMPLOYEE GOT HIS LEFT 5TH FINGER CAUGHT BETWEEN TWO PIECES OF METAL. HE RECEIVED TWO STITCHES.
WHILE USING A UTILITY KNIFE THE KNIFE SLIPPED AND CUT THE EMPLOYEES LEFT ARM.
EMPLOYEE WASHING DOWN EQUIPMENT, LET GO OF WATER HOSE, HOSE WAS UNDER PRESSURE AND WHEN EMPLOYEE LET GO OF THE HOSE IT WHIPPED DOWN AND HIT EMPLOYEE ON TOP OF FOOT.
MISSED LAST STEP COMING DOWN OFF LOADER AND FELL.

EE WAS HOLDING A BAR IN LEFT HAND MISSED AND HIT HIS LEFT THUMB CUTTING IT OPEN REQUIRING 7 STITCHES.
EMPLOYEE USED LEG TO PUSH APART FORKS ON SKIDSTEER, HEARD HIS RIGHT KNEE POP.
WHILE AT WORK, EMPLOYEE WAS HOLDING A PRYER BAR AND THE BAR SLIPPED AND CRUSHED HIS RIGHT PINKY FINGER. HE RECEIVED EIGHT STITCHES.
EMPLOYEE WAS WELDING A GATE FOR A SPLITTER BOX. THE LADDER WAS NOT SET UP PROPERLY AND HE FELL.
WAS WORKING ON CONVEYOR AT THE SHAKER DISCHARGE TO CONNECT THE DISCONNECT ELECTRIC BOX UP ABOVE THE FRAME. THE BELT ON THE CONVEYOR CAME ON & PULLED EE INTO THE MOTOR FRAME. HE FELL DOWN INTO THE DROP BOX. THE CONVEYOR STARTED TO SPIN ALLOWING HIM TO PULL HIMSELF ONTO THE DROP BOX.
WHILE TRIMMING A PIECE OF CONVEYOR BELTING ON THE FEEDER, EE CUT THROUGH BELTING AND KNIFE WENT INTO HIS FOREARM.
EE LIFTED A 20' PVC PIPE WITH A STEEL 90 DEGREE ELBOW AND SPRAINED HIS RIGHT SHOULDER.
TRIED TO FREE ROCK BY REACHING DOWN WITH HOOK FROM ABOVE AND PULLED ROCK LOOSE, HE FELT PAIN IN BACK.
WHILE PUTTING A CHAIN AROUND A CONCRETE ROCK, WHEN HE AND A CO-WORKER STARTED TO PULL THE ROCK UP IT SMASHED RIGHT HAND BETWEEN CHAIN AND ROCK.
DIGGING MATERIAL OUT OF HIGHWALL. BUCKET CAUGHT A ROCK AND CAUSED THE LOADER TO JERK, CAUSING EMPLOYEE TO WRENCH HIS BACK.
LIFTING CONVEYOR TABLE AND TURNED BODY AND PULLED MUSCLE. DID NOT USE HOISTING EQUIPMENT THAT WAS AVAILABLE.

WHILE TAKING OLD LINERS OFF OF CONCRETE MIX DRUM ON THE BATCH PLANT, THE AIR HAMMER HE WAS USING JAMMED AND WHEN HE PULLED BACK, THE HAMMER SQUASHED HIS FINGER AGAINST THE DRUM.
